胆管癌组织中E-钙粘附素的表达及其临床意义

摘　　要：目的探讨E-钙粘附素(E-cadherin,E-cd)在胆管癌发展中的作用及与胆管癌浸润、转移和预后的关系。方法应用免疫组织化学法(S-P法)对52例经手术和病理证实的胆管癌及10例经手术切除的正常胆管壁标本进行E-cd蛋白检测。结果1.E-cd表达在胆管癌组织、癌旁组织及正常胆管上皮组织阳性率分别为36.5%(19/52396%(30/31)和100%(10/10),有显著性差异(P<0.01);2.在52例胆管癌组织中,E-cd表达减弱分别与肿瘤的病理分级、TNM分期、浸润和转移及手术方式有关(P<0.05),而与患者性别、年龄、肿块部位及大小无关(P<0.05);3.E-cd表达阳性患者一年生存率明显高于阴性者(P<0.01)。结论E-cd表达与胆管癌发展、浸润和转移密切相关,检测E-cd的表达有助于胆管癌转移潜能和预后的判断。Objective To study the expression of E-cadherin in the progression of cholangiocarcinoma, and the relationship between the exprssion of E-cd and invasion, metastasis and prognosis. Methods The expression of E-cd in 52 cases of cholangiocarcinoma was examined with S-Pimmunohistochemical technique. Results The positive rate of E-cd expression in cholanguocarcinoma, tumor-adjacent and normal epithelium of bile duct was 36. 5% （ 19//52 ） ,96% （30/31 ）and 100% （ 10/10 ）, respectively （P 〈 0. 01 ）2. In 52 cases of cholangiocarcinoma, the significant correlation was observed between low E-cd expression and pathologial grade, TNM stage, invasion, metastasis and the methods of operation （ P 〈 0. 05 ）, and ono difference could be found between E-cd expression and sex ,age of patients, site and size of tumor（ P 〈 0.05 ）. 3. The one-yeatg survival of patients with posityive E-cd expression was significantly higher than that of patients with low or negative expression （ P 〈 0.05 ）. Conclusions The expression of E-cd may be linked with progression and invasion, Detemination of E-cd will help in perdictiing tumor metastatic potential and prognosis of patients with cholangiocatcinoma.
