Twin-block矫治器矫治AngleⅡ类1分类错畸形的探讨

摘　　要：目的 介绍Twin—block矫治器制作要点，比较应用Twin—block矫治器矫治AngleⅡ类1分类错[牙合]畸形患者矫治前后牙及骨骼的变化。方法 选择生长发育以下颌后缩为主的AndeⅡ类1分类错[牙合]畸形患者8例，磨牙均为远中关系，男5例，女3例，平均年龄（10±1．6）岁，采用Twin—block矫治器矫治10～11个月，测量矫治前后X线头颅侧位定位片各项指标的变化。结果 矫治10～11个月后，患者面型明显改善，磨牙为中性关系，上前牙前突减小，覆盖关系正常。X线头影测量的结果，牙齿的改变主要是角U1-L1增大，平均增大7．25°；U1-NA角减小，平均小5．5°；L6-MP的增大，平均增大3．68mm，颌骨的改变，主要是角SAB增大，平均增大4°；角ANB减小，平均减小4．5°。结论 Twin—block矫治器矫治可使上下牙槽骨得到改建，并诱导下颌功能性前移，促进下颌生长发育，从而协调上下颌关系，改善面型。Objective To introduce the main points of using Twin-block appliance , and evaluate the dentoalveolar and skeletal changes caused by Angle class Ⅱ division 1 malocclusion correction in the patients treated with Twin-block appliance. Methods Eight patients （male 5,female 3）in growth with Angle class Ⅱ division 1 malocclusion and mainly with mandibular retrognathia were investigated, average age being 10 ± 1.6 years old . Lateral cephalograms in each cases were performed before and after treatment , and statistical analysis was done with computer. Results After 10 to 11 months treatment ,the appearance and profile improved significantly. the overjet and overbite normal, molars were rejected into centric occlusion. X-ray cephalometry before and after treatment were compared . U1-L1 angle was increased by an average of 7.25°, U1-NA angle was reduced by an average of 5.5°,L6-MP was increased by an average of 3.68.In addition ,SNB angle was increased by an average of 4°, ANB angle was reduced by an average of 4.5° （ P 〈 0.05）. Conclusion Twin-block appliance possesses the orthopedic effect on Angle class Ⅱdivision 1 malocclusion. The mechanism of orthopedics is to reposit the mandible forward, and to stimulate mandibular growth. Simultaneously it causes the changes of dento-alveolar. All these aspects contribute to a harmonious relationship of upper and lower jaws and a better profile.
