重复扩张术修复大面积颊部皮肤缺损的应用

摘　　要：目的总结重复扩张术修复大面积颊部皮肤缺损的应用效果,探索较佳的治疗方法。方法对107例颊部皮肤缺损面积为7cm×8cm～10cm×12cm的患者,采用皮肤重复扩张术治疗。其中81例患者进行了2次扩张,26例患者进行了3次扩张,每次置入新的扩张器及扩张的间隔时间最少6个月。结果107例患者皮肤缺损全部修复,表情、功能及外观形态恢复良好。但经3次扩张的部分患者出现不同程度的眉、外眦下移、睑外翻及颊部出现不同程度的凹陷。结论重复扩张术是修复颊部大面积皮肤缺损的一个可行的方法,但是要注意扩张次数和并发症之间关系,尽量减少扩张次数,以少于3次为宜。Objective To summarize the treatment of large cheek skin defect by serial tissue expansion, and to explore a better scheme for treatment. Methods 107 patients with large area of cheek shin de- fect ranging from 7cm×8cm～10cm×12cm were treated by serial tissue expansion. Twice tissue expansion was performed on 81 cases, three times was on 26 cases. The interval between each time of expansion was at least 6 months. Results All the 107 cases were satisfiedwith the defect.repair, expression recovery and natural contour. But in the patients with three times expansion, a few complications such as different degrees of lower eyelid ectropion and flatten cheek, drooping of lateral canthus occurred. Conclusion The serial tissue expansion is a feasible method for the large skin defect of check, but the relation between the times of tissue expansion and the complications should pay attention to. The times of tissue expansion should be limited within 3 times.
