模拟生理波电刺激促进损伤周围神经的功能修复

摘  要:目的:观察模拟生理波电刺激对周围神经损伤修复后神经再生的促进作用。方法:实验于2003-11/2004-07在山东大学动物实验中心实验室进行。将160只SD成年雄性大鼠制成坐骨神经移植的模型,单纯随机分为4组(n=40):①对照组:不干预。②甲钴胺组:腹腔注射甲钴胺125μg(商品名弥可保,沈阳卫材制药有限公司生产,批号:031101),1次/d,持续2~20周。③电刺激组:给予持续时间1.2~1.5ms的模拟生理波型波,频率为1Hz,1次/d,15min/次,治疗2~20周。④甲钴胺+电刺激组:同时给予甲钴胺和模拟生理波电刺激,剂量和方法同上。于术后2,4,8,16,20周每组随机取8只大鼠,测定坐骨神经功能指数、神经传导速度、小腿三头肌湿质量等指标。结果:160只大鼠全部进入结果分析。①坐骨神经功能指数:术后2周,对照组为-99.30±8.34,显著低于其他3组(P<0.05,0.01),甲钴胺组低于电刺激组和甲钴胺+电刺激组(-90.08±2.72,-81.96±3.64,-81.96±2.94,P<0.05);术后4,8,16,20周,对照组明显低于电刺激组和甲钴胺+电刺激组(P<0.01),甲钴胺组仍低于电刺激组和甲钴胺+电刺激组(P<0.05),电刺激组和甲钴胺+电刺激组各时间点差异均不显著(P>0.05)。②神经传导速度:术后2,4,8,16,20周,对照组低于甲钴胺组,甲钴胺组低于电刺激组和甲钴胺+电刺激组(P<0.05),电刺激组和甲钴胺+电刺激组各期比较差异均不显著(P>0.05)。③小腿三头肌湿质量:术后2周,对照组和甲钴胺组低于电刺激组和甲钴胺+电刺激组犤(0.66±0.15),(0.70±0.13),(1.11±0.26),(1.12±0.14)g,P<0.01犦;术后4,8,16,20周,对照组低于其他3组(P<0.05,0.01),甲钴胺组低于电刺激组和甲钴胺+电刺激组(P<0.05),后2组之间各时间点比较差异不显著(P>0.05)。结论:模拟生理波电刺激能加速神经的修复再生,效果优于甲钴胺。AIM: To observe the promotion of simulated physiological wave electric stimulus to the regeneration of injured peripheral nerve after repaired. METHODS: The experiment was conducted at the animal experimental center of Shandong University from November 2003 to July 2004. 160 adult male SD rats were made into models of sciatic nerve transplantation, which were randomly divided into 4 groups with 40 rats in each group:① control group: not given any intervention; ②mecobalamine group: The rats were intraperitoneally injected with 125 μg mecobalamine (commercial name: mikebao, produced by Shenyang Weicai Pharmaceutical Co., Ltd.,No. 031101) once daily for 2 to 20 weeks;③electric stimulus group: The rats were given the simulated physiological wave of 1.2-1.5 ms with the frequency of 1 Hz, once daily, 15 minutes every time for 2-20 weeks; ④ mecobalamine + electric stimulus group: The rats were given the intraperitoneal injection of mecobalamine and simulated physiological wave eldctric stimulus at the same dose and the same method, Eight rats of each group were selected to determine the index of sciatic nerve functional index (SFI), nerve conduction velocity and wet weight of triceps surae at 2, 4, 8, 16 and 20 weeks after operation, respectively. RESULTS: All the 160 rats were involved in the result analysis. ①SFI: After 2 weeks of operation, SFI of the control group was -99.30±8,34, significantly lower than the other three groups (P 〈 0.05, 0.01), the mecobalamine group was lower than the electric stimulus and mecobalamine + electric stimulus groups (-90.08±2.72, -81.96±3.64, -81.96±2.94, P 〈 0.05); after 4, 8, 16 and 20 weeks, the control group was obviously lower than the electric stimulus and mecobalamine + electric stimulus groups (P 〈 0.05); the mecobalamine group was still lower than the electric stimulus and mecobalamine + electric stimulus groups, and there was no significant difference between the electric stimulus and mecobalamine + elect
