草酸铂联合氟尿嘧啶、醛氢叶酸治疗晚期结直肠癌的临床观察

摘　　要：目的：观察草酸铂（L—OHP）联合氟尿嘧啶（5-FU）、醛氢叶酸（LV）作为一线方案治疗晚期结直肠癌的临床疗效和可耐受性。方法：23例晚期结直肠癌患者应用5-FU500mg／d持续静脉滴注，第1—5和第8-12天；醛氢叶酸（LV）100mg／d静滴，第1、8天；叶酸片60mg／d1：3服，第2—5、9—12天；草酸铂65mg／m^2静滴，第1、8天，21天为1周期，连用2周期以上评价疗效。结果：23例均可评价疗效，CR2例，PR10例，总有效率（CR＋PR）47．8％。毒副作用主要为1—2度恶心、呕吐、腹泻、口腔溃疡、末梢神经异常及骨髓抑制。结论：L-OHP／联合5-FU／LV方案治疗晚期结直肠癌疗效确切，耐受性好，值得临床进一步研究和推广。Objective：To observe the efficacy and tolerability of continuously infusing fiuorouracil （5-FU） /folic acid combined with oxaliplatin （L-OHP/5-FU/LV regimen） as first line treatment in advanced colorectal cancer. Methods：23 patients of advanced colorectal cancer were treated with 5-FU 500rag/d, civ, d1 -d8 ,d8 - d12, leucoverin 100mg/d, iv gtt, d1,d8, folic acid tablet 60mg/d, po, d2 -d5 ,d9 -d12, oxaliplatin 65mg/（ m2 · d）, iv gtt, d1 ,d8 ,repeated every 21 days（one cycle）. The effect was evaluated after two cycles. Results：Complete response in 2 cases and partial response in 10 cases were observed with an overall response rate of 47. 8%. Adverse effects were mainly grade Ⅰ -Ⅱ , including nausea, vomiting, diarrhea, mouth ulcer, peripheral neuritis and marrow Inhibitory. Conclusion：L-OHP/5-FU/CF regimen is an effective and better tolerated alternative treatment in advanced colorectal cancer and yields promising clinical application.
