摘 要:目的:通过实验对比口腔鳞癌(OSCC)和口腔正常黏膜中乙酰肝素酶(Hpa)、碱性成纤维细胞生长因子(Factor,bFGF)的表达和微血管密度(MVD)的关系,从而了解OSCC中Hpa在肿瘤血管生成中的作用,为OSCC的研究及其治疗提供理论依据。方法:选用正常人口腔黏膜组织10例,经手术切除及活检的OSCC标本48例,采用免疫组织化学的方法检测Hpa、bFGF的表达,用CD31抗体进行血管内皮染色,计数微血管密度,分析Hpa、bFGF在不同组织中的表达、其相互关系及二者与MVD和OSCC组织分化程度、浸润深度、淋巴结转移的关系。结果:(1)在OSCC组织中Hpa、bFGF的阳性表达分别为35例(72.92%)、38例(79.17%),均显著高于在正常口腔黏膜组织中的表达(P<0.05);(2)Hpa表达与肿瘤的淋巴结转移和浸润深度密切相关(P<0.05),bFGF蛋白表达与淋巴结转移相关(P<0.05);(3)Hpa、bFGF阳性表达的OSCC组织中MVD显著高于阴性表达组(P<0.05)。Hpa和bFGF的表达显著相关(P<0.001),且两者的表达都与MVD呈显著相关(P均<0.001)。结论:Hpa与bFGF在OSCC中同时表达,与血管生成密切相关,对OSCC的生长、浸润和转移有促进作用,Hpa可作为反映OSCC生物学行为的客观指标。Objective: To compare the expression of heparanase (Hpa) in normal tissue and oral squamous-cell cancer (OSCC), and to investigate the correlation among the expression of Hpa, and basic fibroblast growth factor ( bFGF), and the microvessel density (MVD) in OSCC, thereby, to probe the function of heparanase in angiongenisis, so as to provide valuable scientific basis for the research and treatment of OSCC. Methods: The expression of Hpa and bFGF in 48 cases of human OSCC and 10 specimens of normal oral mucosa was studied by using conventional immumohistochemistry method, and the endothelia MVD was observed with immumohistochemistry staining using anti-CD31 antibody. The correlations were analyzed. The connection between the expression of Hpa and bFGF in different tissues, and the differentiation, invasion depth, and lymph node metastasis of OSCC was assessed. Results: ( 1 ) Positive expression ratios of Hpa, bFGF in OSCC tissues were 72.92% (35/48) and 79.17% (38/48) respectively. Their expressions were significantly higher in OSCC than those in normal oral mucosa ( both P 〈 0.05 ) ; ( 2 ) Hpa expressions were closely correlated with tumor' s lymph node metastasis and invasive depth ( all P 〈 0. 05 ). The expression of bFGF correlated with lymph node metastasis ( P 〈 0. 05 ). (3) The expression of Hpa and bFGF positively correlated with MVD. The MVD of OSCC tissues with positive expressions of Hpa and bFGF was significantly higher than that of OSCC tissues with negative expressions ( P 〈 O. 05 ). Conclusions: Hpa and bFGF that co -expressed in OSCC correlate with angiogenisis. They may play a key role in growth, invasion and metastasis of OSCC. Hpa may be used as an index reflecting the biological characteristics of OSCC.
