血浆胱氨酸蛋白酶抑制剂C水平与脑出血关系的病例对照研究

摘　　要：目的探讨血浆胱氨酸蛋白酶抑制剂C（胱抑素C）与脑出血的关系。方法收集94例脑出血患者为脑出血组，选择94例性别、年龄、居住地等与脑出血组匹配的非脑卒中者为对照组，用颗粒增强免疫比浊法测定血浆胱抑素C水平，按其水平由低到高将研究人群分为四等份，用logistic回归分析得出各四分位和脑出血组的关系，用相关检验分析胱抑素C水平与脑出血组其他危险因素的关系。结果脑出血组患者血浆胱抑素C水平与对照组无显著性差异[（1．07±0．35）mg／LUS（1．02±0．48）mg／L，P〉0．05]．胱抑素c与第1个四分位比较，第2、3、4个四分位的危害比和95％C，分别为1．58（0．59～4．19）、3．71（1．40～9．80）、2．64（1．01～6．91）；胱抑素C与同型半胱氨酸、肌酐和尿微量白蛋白呈正相关，r分别为0．145、0．288和0．378．结论血浆胱抑素C水平升高是脑出血的一个危险因素。Objective To investigate the association of the plasma Cystatin C levels and cerebral hemorrhage. Methods Ninety-four cases of cerebral hemorrhage were collected and 94 cases without cerebral hemorrhage matched for gender, age, and residence served as control. A particle-enhanced immunonephelometric assay with a nephelometer （BNII, Dade Behring） was employed to measure the plasma concentration of Cystatin C. The subjects were divided into 4 quartiles in order of increasing Cystatin C level. The relationship between plasma Cystatin C level and cerebral hemorrhage was analyzed by logistic regression and the correlations with other parameters were also evaluated. Results There was no significant diffcrence between the plasma Cystatin C levels in cerebral hemorrhage patients and controls [ （ 1.07± 0.35） mg/L vs （ 1.02 ± 0.48） rag/L, P 〉 0.05 ], but high Cystatin C levels were directly associated with high risk of cerebral hemorrhage. As compared with the first quartile, the hazard ratios （and 95 percent confidence intervals） for cerebral hemorrhage were as follows： second quartile, 1.58（0.59 to 4.19）; third quartile, 3.71（1.40 to 9.80）：fourth quartile, 2.64 （1.01 to 6.91 ）. Cystatin C level was positively correlated with homocysteine, urine microalbumin and plasma creatinine. Conclusion High level of Cystatin C is a risk factor for cerebral hemorrhage.
