摘  要:目的探讨单光子断层显像(SPECT)在急性淋巴细胞白血病患儿应用柔红霉素化疗后左心功能测定中的应用价值。方法用SPECT门控心血池显像及超声心动图对15例在我院确诊为急性淋巴细胞白血病患儿应用柔红霉素化疗后(治疗组)心功能的改变进行测定,并与对照组比较。结果15例患儿应用柔红霉素后,SPECT检测结果与对照组比较示左心室收缩功能降低(P<0.05),左心室舒张功能、右心室收缩功能无明显改变(P>0.05)。超声心动图检查治疗组与对照组比较无明显改变(P>0.05)。结论急性淋巴细胞白血病患儿应用柔红霉素后可导致左心室收缩功能减退。SPECT能早期、无创、动态监测心功能情况,是评价柔红霉素治疗急性淋巴细胞白血病患儿左心功能状态的敏感、可靠指标。Objective To evaluate the applied value of single photon emission computed tomography (SPECT) in detecting the effect of Daunorubicin on left heart function in children with acute lymphoblastic leukemia. Methods The changes of heart function in 15 children with acute lymphoblastic leukemia,who were treated with Daunorubicin,were detected by single photon emission computed tomography(SPECT) cardiac blood pool imaging and echocardiography. Results The results of SPECT showed that systolic function of left ventricle was reduced ( P 〈 0.05 ) while the diastolic function and the systolic function of fight ventricle were not changed significantly ( P 〉 0.05 ) after the children with acute lymphoblastic leukemia were treated with Daunorubicin. The results of the echocardiography showed no obvious change compared with those of the control group ( P 〉 0.05 ). Conclusion Daunorubicin can cause contractile hypofunction of left ventricle. SPECT can be used to detect the function of the heart early and dynamically without injurlng,which is sensible and reliable to evaluate the state of the left ventricle function in children with acute lymphoblastic leukemia treated with Daunorubicin.
