摘 要:①目的探讨改良极低蛋白饮食(VLPD)联合α-酮酸对慢性肾功能不全患者透析前营养状况的影响。②方法测定患者在应用改良极低蛋白饮食12、24、48周后,BUN、C r、Hb、TP、A lb、TLC、BH、BW、MAC、TSF以及SGA等参数变化的情况。③结果VLPD治疗后,按SGA法评价患者实验T组营养良好的人数远高于阳性对照C组与阴性对照N组(P<0.01)。BM I、MAMC、ALB在T组中有上升趋势,N组中有下降趋势(P<0.05)。各组TLC、Hb变化趋势不甚明显(P>0.05)。BUN、C r在T组似与C组接近,比较平稳,优于N组(P<0.01)。T组肾死亡及失访率远远低于C组、N组(P<0.01)。④结论改良VLPD治疗慢性肾功能不全具有改善并维持患者营养状况以及延缓肾功能衰竭的趋势。Objective To determine ameliorate predialysis status in patients with chronic renal insufficiency treated with a improved very low protein diet(VLPD) combined with Ketoacids. Methods An improved VLPD was.given during the 48 weeks. BUN ,Scr, Hb,TP, AIb,TLC, BH, BW, MAC, TSFand SGA were measured before and after VLPD 12,24 and 48 weeks. Results After treatment, there was an increasing tendency in BMI, MAMC ,ALB in Group T ( P 〈 0.05 ) ,on the contrary, a reductional tendency in Group N ,the well nourished cases in the Group T were significantly increased (P 〈0.01 ) ; Hb, TLC had no change. BUN,SCR were stable in Group T and C, compared with Group N(P〈0.01 )o The lost visiting rate and renal death event rate in Group T was significantly lower than it in Group N and C (P 〈 0.01 ) . Conclusion The improved VLPD may help to maintain and improve the nutritional status of the patients with chronic renal insufficiency,help to reduce the renal death event rate.
