热疗联合奥沙利铂与5-Fu/CF治疗晚期结直肠癌

摘　　要：【目的】观察局部射频透热联合化疗治疗晚期结直癌的临床疗效和不良反应。【方法】62例晚期结直肠癌患者分为化疗组和联合治疗组,化疗组以奥沙利铂加5-Fu/CF化疗,方案为奥沙利铂100mg/m2d1,5-Fu 500 mg/m2d3、d4、d6、d7、d9、d10/CF 80 mg/m2d3、d4、d6、d7、d9、d10,每3周一个疗程;联合治疗组30例行局部射频热疗配合化疗,化疗方案同化疗组,热疗在d1、d4、d7、d10进行,2个疗程后进行疗效评价。【结果】联合治疗组与化疗组的有效率分别为70%、43.75%(P<0.05),中位生存期分别为11个月与8个月(P>0.05),1年生存率分别为36.67%与28.13%(P>0.05)。两组不良反应无明显差异。【结论】射频透热可提高奥沙利铂加5-Fu/CF的对晚期结直肠癌的疗效,射频透热联合化疗安全有效。[Objective]To evaluate the effects and side effects of the radiofrequency hyperthermia combined with oxalipatin and 5-Fu/CF in treating advanced colorectal cancer. [Methods]There were six-two patients of advanced colorectal cancer were divided into thermochemotherapy and chemotherapy groups. The chemotherapy regimen of 32 cases was that oxalipatin 150 mg iv by drip dt , 5-Fu 750 mg iv by drip d3, d4, d6 , d7, d0 , d10/ CF100 mg iv by drip d3 ,d4 ,d6 ,d7 ,d9 ,d10 , 3 weeks in a cycle,3 cycles were carried out. There were 30 patients in thermochemotherapy group, the fractional radiofrequency thermotherapy was carried out on d1, d4 ,d6, d10. Therapeutic effect was evaluated after 2 courses of treatment. [Results] The effective rate was 70% and 43. 75%,median survival time （MST） was 11 months and 8 months, 1 year survival rate was 36.67% and 28. 13% in theromotherapeutic group and chemotherapy group respectively. There were significant differences in effective and survival rates between two groups. No significant differences of the side effects were observed in both groups. [Conclusion]Radiofrequency hyperthermia combined with oxalipatin, 5-Fu/CF systemic chemotherapy can reach a higher response rate than chemotheraphy alone in patients with advanced colorectal cancer and it＇s thermochemothery is safe.
