清肝化瘀方药对原发性肝癌患者的疗效及对细胞免疫功能的影响

摘　　要：目的探讨清肝化瘀方药对原发性肝癌患者的疗效及对细胞免疫功能的影响。方法原发性肝癌患者80例,随机分为两组;治疗组50例,服用清肝化瘀方药,对照组30例给予复方氟脲嘧啶多相脂质体口服液,观察临床疗效和外周血细胞因子白细胞介素2(IL-2)、白细胞介素12(IL-12)、肿瘤坏死因子α(TNF-α)、转化生长因子1β(TGF-β1)和γ-干扰素(IFN-γ)的变化。结果清肝化瘀方药能改善患者细胞免疫功能,有效率达36.0%,对照组为10.0%,两组比较差异有统计学意义(P<0.01);服用清肝化瘀方药前与用药后相比TGF-β1(0.69±0.06 vs 0.58±0.09)ng/LI、L-12(48.87±10.21 vs 28.58±18.39)ng/L、TNF-α(263.16±29.02 vs 217.78±30.56)ng/L明显下降(P<0.05);IL-2(1.47±0.33 vs 1.89±0.38)ng/L、IFN-γ(39.10±12.21 vs 54.77±6.63)ng/L明显升高(P<0.05)。结论清肝化瘀方药是治疗肝癌有效的药物。Objective To demonstrate the alterations of therapeutic effect and cellular immunological function of Qinggan decoction in patients with primary liver carcinomas（PLC）. Methods The patients with PLC were randomly divided into treatment group （50 cases） given Qinggan decoction or control group （30 cases） who took compound fluxuridine. The serum levels of interleukin-2 （IL-2）, interleukin-12 （IL-12）, tumor necrosis factor α （TNF-α）, transforming growth factor-β1 （TGF-β1） and interferon γ（IFN-γ） were measured by enzyme-linked immunosorhent assay （ELISA） after administering. Results Qinggan decoction significantly enhanced the cellular immunological function. The total alleviation in treatment group accounted up to 36.0%, while that in control group accounted for 10.0% ,showing statistic difference（ P 〈0. 01）. Qinggan decoction could up-regulate the helper T lymphocyte 1 （Th1） cytokines such as IL-2 （1.47±0.33 vs 1.89±0.38） ng/L,IFN-γ （39. 10±12.21 vs 54.77±6.63） ng/L.downregulate the helper T lymphocyter 2（Th2） cytokines TGF-β1 （0.69±0.06 vs 0. 58±0. 09） ng/L,IL-12 （28.58± 18.39 vs 48.87±10.21） ng/Land TNF-α （263.16±29.02 vs 217.78±30.56）（ P 〈0. 05）.Conclusion Qinggan decoction is an efficacous drug in the treatment of primary liver carcinomas.
