脑胶质瘤光动力学治疗方法及疗效观察

摘　　要：目的 利用光敏素对43例脑胶质瘤患者进行激光光动力学治疗，观察治疗效果。方法 脑胶质瘤切除术前24h内，静脉滴注光敏素。术中行肿瘤根治术或尽量切除肿瘤组织，并于术中行手术残腔内激光照射、留置光纤，术后24h通过光纤再次行激光照射治疗。结果 随访5～48个月，影像学CT和（或）MRI证实无瘤生存22例，肿瘤复发18例，死亡3例。无术后持久神经功能缺失。结论 在脑胶质瘤患者中采用光敏素激光光动力学治疗能明显延长患者无瘤生存期，是治疗恶性脑胶质瘤的有效方法。Objective To evaluate the effect of photodynamic therapy in patients with glioma.Methods Fortythree patients with glioma were treated with gross resection and photodynamic therapy. Phytochrome was given to patients by intravenous injection twentyfour hours before resection. During the operations, gross total resections were strived as much as possible. Intraoperative photodynamic therapy was performed with laser in the resective cavity. Twentyfour hours later, postoperative therapy was given to patients through optical fiber. Results For all patients, follow-up for 5 to 48 months. Twenty-three patients survived imageologic tumor free. Recurrence of glioma was confirmed in eighteen patients. Three patients died. No case with persistent neurological deficit was recorded. Conclusions Prolonging tumorfree survival period of patients with glioma, photodynamic therapy was an effective measure for glioma.
