颈外动脉灌注化疗治疗中晚期头颈部鳞癌的近期疗效观察

摘　　要：目的:探讨颈外动脉灌注化疗治疗中晚期头颈部鳞癌的疗效及影响疗效的因素。方法:本组67例患者,全部经病理学检查证实为头颈部鳞癌患者。采用Se ld inger技术,选择性地将导管插入颈外动脉的肿瘤供应支,经导管分次灌注化疗药物。结果:67例患者近期临床疗效总有效率为73.1%。其中多次治疗疗效(79.4%)优于单次治疗(66.7%),多血管型肿瘤的疗效(76.7%)优于少血管型肿瘤(66.7%)。结论:中晚期头颈部鳞癌患者,采用颈外动脉灌注化疗疗效显著,全身不良反应小,可以作为中晚期头颈部癌瘤综合治疗的一种重要治疗手段。治疗中要注意肿瘤的血供类型及强调肿瘤的治疗次数。Objective To explore the effect of the perfusion chemotherapy via external carotid artery on squamous epithelial carcinoma on head and cervical part at metaphase and advanced stage and the factors that influence the effect. Methods Sixty-seven patients were pathologically proved squamous epithelial carcinoma on head and cervical part. Seldinger technique was adopted to insert a catheter into the tumor supplying branch of external carotid artery and perfuse chemotherapy medicine via the catheter. Results The effective rate was 73.1%. The effect treated many times was better than the one treated once, and the effect treated on the tumor rich in blood vessels was better than the one treated on the tumor poor in blood vessels. Conclusion The effect of the perfusion chemotherapy via external carotid artery at metaphase and advanced stage is obvious and the adverse effect on the whole body is slight ,which can be an important technique for malignant tumor on head and cervical part in metaphase and advanced stage. The type of the supply blood vessels for the tumor should be paid more attention and the therapeutic frequency on the tumor must be stressed.
