P300对颅脑外伤所致精神障碍的认知功能评价

摘　　要：目的探讨颅脑外伤所致精神障碍认知功能障碍的特点及P300对其评价的临床价值。方法对56例颅脑外伤所致精神障碍患者和52例颅脑外伤不伴精神障碍患者以及53例健康者分别进行了P300电位测定和智商测定,并将3组结果加以比较。结果颅脑外伤所致精神障碍患者组与无精神障碍组P300电位成分中N2、P3波潜伏期延长和P3波幅降低,言语智商、操作智商、总智商得分均降低,较健康对照组差异有显著性意义(P<0.01),而颅脑外伤所致精神障碍患者组与无精神障碍组相比,P300电位成分中N2、P3波潜伏期延长和P3波幅降低,言语智商、操作智商、总智商得分均降低,差异有显著性意义(P<0.01),相关分析显示P300潜伏期与言语智商、操作智商、总智商得分呈显著负相关(r=-0.29～-0.33,P<0.01),波幅则呈显著正相关(r=0.31～0.36,P<0.01)。结论P300电位可作为评价颅脑外伤所致精神障碍患者认知状况的重要指标应用于临床。Objective To explore the features and the value of P300 potentials in appraising the cognition with psychogenia of head injury patients. Methods P300 potentials test and Wechsler Adult Intelligence Scale（WAIS） test were used for the fiftysix psychogenia of head injury patients and fifty-two nonpsychogenia of head injury patients and fifty-three normal healthy peoples,the results were compared in these three groups. Results The latency of N2 and P3 of p300 potentials in psychogenia of head injury group and nonpsychogenia of head injury group were longer than those in normal control group,the amplitude of P3 of p300 potentials and verbal intelligence quotient （VIQ）, performance intelligence quotient （PIQ）, full intelligence quotient（FIQ） in psychogenia of head injury group and nonpsychogenia of head injury group were lower than that in control group. There were significant difference between the two groups（ P 〈0. 01 ）. Correlation analysis showed that there was a remarkable negative correlation of p300 latency and VIQ ,PIQ and FIQ in WAIS respectively（ r = -0. 29 - -0. 33, P 〈 0. 01 ）, and a significant positive correlation with p300 amplitude respectively（ r = 0. 31 -0. 36, P 〈 0. 01 ）. Conclusions P300 potentials can be used in clinical as a momentous ideal in appraising the cognition with psychogenia of head injury patients.
