摘  要:目的:认识电针治疗缺血缺氧性脑病的远期疗效及可能机制。方法:实验于2004-03/2005-12在咸宁学院医学院神经组化研究室完成。将38只出生7~8d的Wistar大鼠抽签随机分为3组:假手术对照组10只、缺血缺氧组14只、电针治疗组14只。将动物在清醒状态下实施左侧颈总动脉扎结,向伤口处注射青霉素以预防感染,缝合伤口。术后动物在室温中恢复1~6d后,进行低氧处理,将动脉置于恒温37℃的常压缺氧舱内,以1.5L/min的速度通入体积分数为0.08氧和体积分数为0.92N2的混合气体,2h后将动物取出。假手术对照组只作手术,不结扎血管,不缺氧;缺血缺氧组缺血缺氧后不加任何治疗;电针治疗组缺血缺氧后第2天开始,用直径1.5寸毫针,沿皮直刺“百合”“大椎”两(,施以连续波、频率16Hz,强度8V,留针10min,1次/d,同时间进行,10d为1疗程,共2疗程,两疗程间隔2d,(位定位参照大鼠常用的针灸(位,“百合”(在颅顶正中,“大椎”(在背部正中第7颈椎与第1胸椎间。常规饲养20d后,取左侧脑组织切片进行尼氏染色和脑源性神经营养因子免疫组织化学染色。结果:38只大鼠均进入结果分析。①假手术对照组脑组织神经胞浆中布满着深蓝色呈颗粒状的尼氏体。缺血缺氧组神经元内尼氏体大量脱失,缺血区内有大量的空白区域,存在的尼氏体着色浅淡。电针治疗组神经元内尼氏体有一定程度的脱失,但较缺血缺氧组明显减轻。②假手术对照组脑源性神经营养因子免疫阳性细胞较少,缺血缺氧组免疫阳性细胞数明显增加。电针治疗组脑源性神经营养因子表达较缺血缺氧组显著增加,差异显著(皮质区:31.37±9.94,24.51±8.32,海马区:27.64±7.16,21.95±5.13,P均<0.05)。结论:电针对缺血缺氧后脑组织具有神经保护效应,其机制可能与脑源性神经营养因子表达增加有关。AIM: To study the prospective efficacy and possible mechanism of electroacupuncture (EA) on the hypoxia-ischemia (HI) encephalopathy. METHODS: The experiment was carried out in the Department of Nerve Histochemistry, School of Medicine in Xianning College from March 2004 to December 2005. Thirty-eight Wistar rats of 7-8 days old were divided into 3 groups randomly: sham group (n=10), HI group (n=14) and EA group (n=14). All the animals were ligated in left common carotid artery in consciousness, and the wound was sutured after penicillin injection in order to prevent, the infection. After recovery for 1-6 days postoperatively at room temperature, all the animals were managed in hypoxia, and the arteries were placed at constant 37℃ in normal pressure and hypoxia cabin, which was inhaled with the mixed gas of oxygen in 0.08 volume fraction and N2 in 0.92 volume fraction at the speed of 1.5 L per minute. Two hours later, the animals were taken out. Sham group only performed the operation without vessel ligation and hypoxia; HI group did not receive any management after HI; According to the common acupoints, EA treatment was conducted in the rats of EA group simultaneously and directly at Baihui (DU 20) in the center of calva and Dazhui (DU ld) between the 7^th cervical vertebra and the 1^st thoracic vertebra in the dorsal center) with the needle of 1.5 cun in diameter at the continuous wave of 16 Hz in frequency and 8 V in intensity, for 10 minutes once daily from the second day of HI. The management duration was 10 days as a course with the interval of 2 days, totally for 2 courses. After 20 days of regular feed, the left brain tissue sections were obtained for Nissl's staining and brain-derived neurotrophic factor for immunohistochemical staining.
