瞳孔温热疗法在临床应用的疗效观察

摘　　要：目的:观察经瞳孔温热疗法(Transpupillary Thermotheraphy,TTT)治疗脉络膜新生血管的临床效果。方法:对200例临床诊断脉络膜病变的患者,其中老年盘样黄斑病变(渗出型)182例,脉络膜血管瘤8例,中浆合并脉络膜新生血管10例进行TTT治疗及1￣15个月随访观察。结果:老年盘样黄斑病变110例复诊,平均随访1年。其中50例脉络膜新生血管萎缩,占复诊患者45%;视力不变者20例,占18%;视力增进者70例,占64%;视力下降者20例,占18%。脉络膜血管瘤5只眼复诊,平均随诊6个月。1只眼瘤体无变化,其余4只眼瘤体萎缩。视力不变者3只眼,增进及下降各1只眼。中浆合并脉络膜新生血管3只眼复诊,黄斑区水肿吸收。结论:TTT对脉络膜疾病临床疗效肯定,副作用少,是一种很有潜力的脉络膜新生血管、血管瘤的治疗方法。Objective To evaluate the clinical effect of wanspupillary thermotherapy（TTT） in the treatment of choridal new vessels. Method 200 patients with choridal disease were treated with TTT and followed up for 1-15 months. Among them, include 182 cases of choridal exudative senile macular degenration, 8 cases of choridal hemangioma, 10 cases of central serous chorioretinopathy combined with choridal new vessel. Results In 110 patients with choridal exudative senile macular degeneration, whose average time of follow-up was 1 year, 50 cases（45%） showed shrunkened choridal new vessel, 20 cases（18%） showed unchanged visual acuity, 70 cases（64%） showed improved visual acuity, and 20 cases （18%） showed declined visual acuity. Among 5 cases of choridal hernangioma （average time of follow-up was 6 months）, the tumor was unchanged in 1, shrunkened in 4, and visual acuity was unchanged in 3, improved in 1, declined in 1. 3 cases of central serous chorioretinopathy combined with choridal new vessel, macular edema were all absorbed. Conclusion TTT is effective for the treatment of choridal disease and has few complications. It is a kind of potential treatment for choridal new vessel and choridal hemangioma.
