鞍上脑膜瘤的显微外科手术

摘　　要：目的：总结近６年来应用显微外科技术切除鞍上脑膜瘤３８例的临床经验。方法：在显微镜下能清楚显露鞍上肿瘤及周围结构，手术中牵拉脑组织轻微，对周围重要结构影响小。结果：肿瘤全切除２８例，次全切除６例，大部分切除４例，术后视力恢复改善２４例。结论：应用显微外科切除鞍上脑膜瘤可明显影响手术效果，提高肿瘤全切除率，降低手术死亡率。Objective:This article reports clinical experience of the microsurgical management of suprasellar meningiomas for 38 patients nearly 6 years,Method:Use of operating microscope can clear magnify suprasellar meningiomas and relationship with the surrounding structures,and help sensure success in the operation with least amount of injury to structures.Results:Among the 38 patients,total removal of the tumor was achieved in 28 patients,subtotal excision in 6 patients,and great partial excision in 4 patients,the visual function was improved in 24 patients.Conclusions: The surgical outcome was significantly influenced using the microscope techniques.The success rate in total tumor resection is greatly increased and the rate of mortality is markedly declined.
