摘  要:目的:探讨疼痛对烫伤大鼠血浆TNFα(肿瘤坏死因子α)以及胰岛素抵抗和胰岛β细胞功能的影响。方法:40只SD大鼠乙醚麻醉烫伤后,随机分为疼痛组(n=20)和麻醉组(n=20)。测定疼痛组和麻醉组大鼠血浆TNFα、空腹血糖、胰岛素、胰高血糖素和皮质醇水平,并观察稳态模式评估法胰岛素抵抗指数(HOMA-IR)和β细胞胰岛素分泌指数(HOMA-β)变化。结果:疼痛组空腹血浆TNFα、空腹血糖、空腹胰岛素、胰高血糖素和皮质醇水平均明显高于麻醉组(P均<0.01)。疼痛组HOMA-IR明显高于麻醉组(P<0.01),疼痛组HOMA-β明显低于麻醉组(P<0.01)。结论:疼痛应激促进烫伤大鼠血浆TNFα升高,并在胰岛素抵抗和β细胞功能不全中发挥重要作用,控制疼痛有助于改善烧伤后高血糖症。Objective: To investigate the effects of pain on plasma TNFα, the insulin resistance and function of the β-cell in the traumatic rats. Methods:The plasma TNFα, fasting blood glucose, the insulin, the glucagon and the cortisoli of 20 scalded SD rats with pain (pain group) and 20 scalded SD rats with anaesthesia(anaesthesia group) were measured and the index of insulin resistance and insulin secretion of β-cell of both the pain and anaesthesia groups were calculated. Results:The plasma TNFα, the fasting blood glucose, the insulin, the glucagon and the cortisoli of the pain groups were higher than that of anaesthesia groups(P〈0. 01); The indexes of the HOMA-IR of the pain group were higher than that of the anaesthesia group(P〈0.01), and the indexes of HOMA-β of the pain group were lower than that of the anaesthesia group(P〈0.01). Conclusion: The hyperirritable pain may play an important role in the traumatic insulin resistance and dysfunction of the β-cell. The treatment with anodyne may be helpful to prevention of the traumatic hyperglycemia.
