复方甘草甜素预防抗结核药物性肝炎的临床研究

摘　　要：目的：探讨复方甘草甜素对抗结核药物性肝炎的预防作用。方法：将我院2004年1月-2005年6月住院的252例肺结核患者随机分为处理组、对照组A和对照组B，处理组予复方甘草甜索和抗结核药物联合治疗，对照组A予肝泰乐和抗结核药物联合治疗．对照组B仅予抗结核药物治疗．观察药物性肝炎的发病情况。结果：加用复方甘草甜索后，84例肺结核息者发生药物性肝炎2例，发病率为2．4％，但病情轻。而对照组A发生药物性肝炎11例，对照组B发生药物性肝炎12例，重症1例，发病率分别为13．1％和14．3％，抗结核药物性肝炎主要发生于强化期，表现为乏力、恶心、呕吐、纳差、黄疸、腹胀、肝痛等。结论：复方甘草甜素可有效预防抗结核药物性肝炎．而肝泰乐不能预防抗结核药物性肝炎。Objective：To explore the preventive effect of glycyrrhizin on the hepatitis suffered from anti--tuberculous drugs. Methods： Retrospective study was adopted. Two hundred and fifty-- two inpatients suffered from hepatitis resulted from anti-tuberculous drugs from Jan,2004 to Jun, 2005 were randomly sorted out into treatment group, control group A and control group B. The treatment group was treated with glycyrrhizin combined with anti-tuberculous drugs, but the control group A was treated with anti-tuberculous drugs and glucurolactone. The control group B was treated only with anti-tuberculous drugs. The incidence of the hepatitis suffered from antituberculous drugs was observed. Results： Two of 84 cases in the treatment group were suffered from the hepatitis,but the state of the two patients wasn＇t severe. Its incidence of anti-tuberculous drugs＇hepatitis was 2.4 %. Otherwise, 11 of 84 cases in the control group A obtained the hepatitis and 12 patients in the the control group B were suffered from the hepatitis,and one of them had severe state of an illness. The incidence of anti-tuberculous drugs＇ hepatitis from the two control groups was 13.1% and 14.3% ,respectively. The hepatitis in 10 patients occurred during the enhanced period and the clinical manifestations presented as tiredness, nausea, vomit, icterus, windy feeling,pain in liver. Conclusion.Glycyrrhizin rather than glucurolactone might play a role in the preventon from anti-tuberculous drugs＇hepatitis.
