翼状胬肉显微手术切除后辅用曲安奈德的疗效观察

摘　　要：目的探讨翼状胬肉显微手术切除后局部应用曲安奈德的疗效及安全性。方法我院1999年8月～2004年12月收治翼状胬肉112例（165只眼），均先行显微手术切除翼状胬肉，然后根据随机分组的原则，将之分为三组。治疗A组：45例（73只眼），曲安奈德5mg于术中、术后2周、术后1月局部球结膜下注射。治疗B组：36例（49只眼），丝裂霉素局部湿敷。对照C组：31例（43只眼）。结果术后随访1年，应用曲安奈德A组、丝裂霉素B组和对照C组翼状胬肉复发率分别为4．1％（3／73）、10．2％（5／49）、27．9％（12／43）；应用曲安奈德A组、丝裂霉素B组和对照C组治疗翼状胬肉并发症发生率分别为1．3％（1／73）、14．2％（7／49）、2．3％（1／43）。治疗结果复发率A组与对照C组比较，x^2检验：x^2AC=11．58，P〈0．01，有显著性差异，说明应用曲安奈德能明显降低其复发率；并发症的发生率比较，曲安奈德与丝裂霉素两组比较，x^2AB=6．01，P〈0．05，有显著性差异，说明应用曲安奈德能明显降低并发症的发生。结论曲安奈德是一种安全、有效降低翼状胬肉术后复发的药物，可以作为翼状胬肉切除术后的常规用药。Objective To evaluate the efficacy and safety of being used triamcinolone acetonide injection （TAI） after micresurgery in pterygium. Methods Aug. 1999 - Dec. 2004, the total cases of 112 （ 165 eyes） with pterygium were divided in three groups by randomized. They were all made micresurgery on pterygium. Group A： 45 patients （73 eyes） , were treated with TAI in operation, 2weeks, 4weeks. Group B：36 patients （49 eyes） , were treated with mitomycin wet dressing. Group C： 31 patients （43 eyes）, no special treated. Results All the patients were followed up and check 1 year. The relapse rate of pterygium in TAI（ Group A）, mitomycin（ Group B） and control groups （Group C） was 4.1% （ 3/73 ）, 10.2% （ 5/49 ）, 27. 9% （ 12/43 ）. Tlere was a significant difference between Group A and Group C （x^2Test： x^2AC = 11.58; PAC 〈 0.01 ）. It shows TAI can obviously reduce pterygium＇ s relapse. The complication in Group A, Group B and Group C was 1.3% （1/73）, 14.2% （7/49）, 2.3% （1/43）. The significant difference compli- cation exists between Group A and B the same way （ x^2 Test：x^2 AB = 6.01 PAB 〈 0.05）. Conclusion The TAI is a kind of saft and efficient medicine for preventing the relapse of pterygium and can be used routinely after microsurgery.
