全直肠系膜切除术治疗中下段直肠癌

摘　　要：目的：探讨全直肠系膜切除术治疗中下段直肠癌的临床疗效。方法：回顾性分析采用保留植物神经的全直肠系膜切除术治疗86例中下段直肠癌的临床资料。结果：全组无手术死亡，平均手术时间为126±42min，术中失血平均235、30±189．50ml，发生吻合口瘘2例（2.33％）。术后拔尿管时间3～14d，平均6.23d。术后4周随访，保肛组67例中51例（76.1％）控便能力良好，术后勃起功能障碍发生率24.1％（14／58），射精功能障碍发生率为27．6％（16／58）。随访时间6个月～9年，平均46个月。发生局部复发2例（2．33％）。术后3、5年生存率分别为81．4％和63．2％。结论：全直肠系膜切除术是预防中低位直肠癌的局部复发。提高生活质量的有效措施。直肠癌根治性手术中采用TME手术方法是完全必要的。Objective：To evaluate the clinical effect of total mesorectal excision on rectal cancer. Methods： Clinical data of 86 cases of rectal cancer were retrospectively analyzed. Results：During operation, no death occured, the average operative time was 126 ± 42 mins, the average volumes of blood loss was 235.30 ± 189,50 ml. Followed up between 6 months and 9 years,local recurrence occured in 2 eases,anastomotic leakage in 2 cases, decreased sexual function in 16 cases. The survival rate of 3 years and 5 years was respectively 81.4% and 63.2%. Conclusion：The total mesorectal excision can prevent local recurrence in rectal cancer. It is effective in improving the life quality of patients. It should be recommended to perform total mesorectal excision on radical rectectomv for rectal tumor.
