肿瘤相关抗原在胃粘膜异型增生组织浸液中表达的研究

摘　　要：目的研究胃粘膜异型增生组织中肿瘤相关抗原表达的临床意义。方法63例患者均行电子胃镜检查,经胃镜取3块病变组织(直径约0.5cm)送病理,据病理诊断分为异型增生组33例,包括中度异型增生组16例,重度异型增生组17例;正常组30例。同时取病变组织2块放入5%EDTA二钠液lml中消化分离细胞,测二组患者病变组织浸液中CEA、CA19-9的含量,同时测血清中CEA、CA19-9的含量。对二组患者组织浸液及血清中CEA、CA19-9的含量及组织浸液/血清比值分别进行比较。结果(1)CEA、CA19-9在异型增生组织浸液中的表达明显高于血清(P<0.01);(2)CEA、CA19-9在异型增生组织浸液中的表达明显高于正常组(P<0.01),且在重度异型增生组织浸液中的表达明显高于在中度异型增生组织浸液中的表达(P<0.01),而血清中肿瘤相关抗原的含量与正常组无显著性差异(P>0.05)。结论病变组织浸液中肿瘤相关抗原表达的检测能预测异型增生的危险程度,提高早期胃癌的诊断率。Objective： objective to study the clinical significance of the expression of tumour related antigen in gastric mucosal dysplasia tissue. Methods ：Sixty three patients diagnosed by Endoscopy and pathology were divided into two groups ,33 patients in group of gastric mucosal dysplasia （16 patients in medial gastric mucosal dysplasia, 17 patients in severe gastric mucosal dysplasia）, and 30 patients in control group. The two little cakes of specimen obtained by forceps biopsy from each patients were put in 1 ml 5% Na2-EDTA solution After digested and dissociated, The tissue extract and serum levels of CEA and CA19-9 in the two group and the ratios of tissue extract/serum were measured and compared. Results： （1） The tissue extract levels of CEA and CA19-9 increased significantly than the serum levels in patients with gastric mucosal dysplasia （P 〈 0. 01 ）. （2） The tissue extract levels of CEA and CA19-9 in group of gartric mucosal dysplasia increased significantly than those in normal group （P 〈0. 01 ） ,and the tissue extract levels of CEA and CA19-9 in patients with severe mucosal dysplasia increased significantly than those with medial mucosal dysplasia. The serum levels of turmor related antigen in group of mucosal dysplasia were no significant differences with those in normal group （P 〉 0. 05 ）. Conclusion：The measure of CEA and CA19-9 in the disease tissue extract can indicate the risk degree of gastric mucosal dysplasia and incease the diagnostic ratio of early stage stomach cancer.
