胃癌患者外周血CEA mRNA的检测及意义

摘　　要：目的探讨胃癌患者外周血中CEA mRNA的表达及临床意义。方法采用巢式RT-PCR技术检测70例胃癌患者手术前、后外周血CEA mRNA的表达情况,并以20例健康志愿者为阴性对照组,以10例新鲜胃癌组织作阳性对照。结果CEA mRNA术前阳性表达率为51.4%,阳性表达率与组织学类型、TNM分期具有显著相关性(P<0.05),而与肿瘤的部位、患者性别无相关性(P>0.05);20例健康志愿者无1例阳性表达,10例胃癌组织均为阳性表达。结论CEA mRNA作为标志物检测胃癌血循环微转移是可靠的,对于指导临床诊断、治疗及判断预后具有重要意义。Objective To study the clinical signifieance of the expression of CEA mRNA in peripheral blood of gastric neoplasm. Methods Seventy gastric neoplasm patients were seleeled in the study. Using nested reverse transeription polymerase chain reaction, we detected the expression of CEA mRNA in the peripheral blood before and after operation. 20 bealtlry donors and the 10 gastric cancer tissue were also included in the studv. Results Of 70 peripheral blood preoperation of gastrie neoplasm patients, the expression positive rate for CEA mRNA was 32.9% , which was significantly correlated with the pathologie differentiation and TNM slates, but not signifieantly with the focus and gender of patients. The expression positive rale of the gene was higher after operation than before operation. It were positive in all gastrie cancer tissne, but negative in peripheral blood of all heahhy people. Conclusion The presenee of CEA mRNA in peripheral blood is a helpful indieator of the hematogenie disseminatoon of tumor cells in patients with gastric cancer, It can be helpful to the judgement of prognosis and selection of treatment methods. Surgical maneuvers were a possible cause of hematogenous metastasis.
