胃癌患者血清肿瘤型M2-PK的测定及其临床意义

摘　　要：目的研究胃癌患者血清肿瘤型M2-丙酮酸激酶(tM2-PK)的含量与临床病理学参数的关系。方法运用ELISA法定量检测tM2-PK在胃癌、胃溃疡、及正常人血清中的含量。结果胃癌患者血清tM2-PK增高,明显大于胃溃疡(P<0·05)与正常人组(P<0·01)。肿瘤≥5cm者明显高于<5cm者(P<0·05);TNMⅢ,Ⅳ期者明显高于Ⅰ,Ⅱ期者(P<0·01);而淋巴结转移情况、肿瘤胃壁浸润深度及组织学分级对tM2-PK水平的影响无明显差异。以大于15U/ml为阳性者,胃癌组阳性率为64%,胃溃疡组假阳性率为20%,正常对照组假阳性率为6%。结论胃癌肿瘤代谢的改变可产生大量tM2-PK。血清肿瘤型M2-PK水平升高可为肿瘤大小和病期提供依据。Objective To determine the serum level of tumor type M2-pyruvate kinase（tM2- PK） in patients with gastric cancer and to elucidate its relationship with clinical characteristics. Methods The serum tM2-PK levels were measured by quantitative sandwich ELISA in 64 gastric cancer patients, 15 gastric ulcer patients and 16 healthy individuals. Results The preoperative serum tM2-PK levels in patients with gastric cancer were significantly higher than those in patients with gastric ulcer（P〈0. 05） and those in healthy individuals（P〈0. 01）. The bigger tumors （≥5 cm） had significant higher tM2-PK than the smaller ones（〈5 cm）（P〈0. 05）. The levels in the patients of stage Ⅲ and stage Ⅳ were significantly higher than those of stage Ⅰ and stage Ⅱ （P〈0.01）. No significant difference could be found in different tumor invasive depth, lymph node metastasis and histological types. Taking 15 U/ml as the positive result, the positive rate was 64 % in gastric cancer patients, and false positive rates were 20% and 6% in gastric ulcer patients and in healthy individuals, respectively. Conclusion With the changing of tumor metabolism, gastric cancer can produce a lot of tM2- PK,which can be measured in serum. The serum levels of tM2-PK may reflect the size and stage of the tumor.
