非小细胞肺癌患者外周血CK19和LUNX mRNA的检测及意义

摘　　要：目的:检测非小细胞肺癌(NSCLC)患者CK19 mRNA和LUNX mRNA的表达情况,探讨其作为微转移检测分子标志物的可行性。方法:选择初治的NSCLC患者62例,以肺部良性疾病10例和健康对照10例作为对照组,采用实时荧光定量逆转录聚合酶链反应(FQ-RT-PCR),检测患者外周血的CK19 mRNA和LUNX mRNA的表达。结果:62例NSCLC患者中外周血CK19 mRNA表达阳性率33.9%(21/62),明显高于非肿瘤组的5%(1/20)(P=0.012);NSCLC组外周血LUNX mRNA表达阳性率29.0%(18/62),明显高于非肿瘤组的0%(0/20)(P=0.007);外周血CK19mRNA和LUNX mRNA表达阳性率与病理分期、病理类型、肿瘤分化程度未见明显相关;外周血CK19 mRNA和LUNX mRNA表达未见明显相关(P=0.376)。结论:CK19 mRNA和LUNX mRNA可作为检测非小细胞肺癌患者微转移特异、敏感的分子标志物,可能有助于早期诊断肺癌转移,从而指导临床分期和治疗。Objective： To detect CK19 mRNA and LUNX mRNA in peripheral blood of patients with non-small cell lung cancer （NSCLC） and discuss the possibility of indicating lung cancer micrometastases by them. Methods： Fluorescence quantitative reverse transcription-polymerase chain reaction （FQ-RT-PCR） was used to detect CK19 mRNA and LUNX mRNA in peripheral blood from 62 patients with NSCLC, 10 patients with benign pulmonary diseases and 10 healthy volunteers. Results： In peripheral blood, the positive rate of CK19 mRNA for NSCLC was 33.9%（21/62） vs 5%（1/20） in controls （P=0.012）, the positive rate of LUNX mRNA for NSCLC was 29.0%（18/62） vs 0%（0/20） in controls （P=0.007）. The positive rate of neither CK19 mRNA nor LUNX mRNA in peripheral blood of NSCLC had a relationship with stage, cell type or differentiation. There was no relationship between CK19 mRNA and LUNX mRNA expression （P= 0.376）. Conclusion： CK19 mRNA and LUNX mRNA are efficient indicating factors on sensitivity and specificity to detect early haematogenous dissemination of cancer cells for patients with NSCLC. This method may lead to an earlier diagnosis of metastasis for lung cancer and do some help to evaluate the cancer more correctly and make the best treatment plan.
