摘 要:目的结合甲状腺良、恶性结节的病理基础,分析其声像图特点,探讨超声对甲状腺良、恶性结节鉴别诊断的临床应用价值。方法对89例甲状腺结节的患者术前行超声检查,术后根据病理结果对其超声征象进行良、恶性对照研究。结果89例甲状腺结节患者中,良性者76例共199个结节,恶性者13例共18个结节,甲状腺结节超声诊断与手术病理结果符合率为81.6%,其中结节性甲状腺肿的诊断符合率为85.2%,甲状腺腺瘤的诊断符合率为83.3%,甲状腺癌的诊断符合率为62.2%。结论超声对甲状腺良恶性结节的鉴别诊断有明确的临床应用价值,综合分析甲状腺良恶性结节的病理基础与超声征象可提高对其鉴别诊断的准确率。Objective Combing the pathological bases with the conventional ultrasound characteristics, we analysed the effectiveness of ultrasound on differentiation of malignant from benign nodules in thyroid gland. Methods Eighty-nine consecutive patients with thyroid nodules who were to undergo surgery were examined by US before thyroidectomy. Results In the eighty-nine cases, seventy-six cases (199 nodules) were diagnosed as benign and thirteen cases (18 nodules) as malignant. The US correlation rate of thyroid nodule with pathology was 81.6 %. Compared with pathology, the US diagnostic accuracy rate for nodular goiters, adenomas and carcinomas were 85.2%, 83.3%, 62.2%, respectively. Conclusions The ultrasonographic differential diagnosis of thyroid malignant and benign nodules is clinically valuable. The combination of conventional US characteristics and pathologic bases in thyroid nodules can improve differential diagnosistic accuracy.
