胃癌患者外周血中五种肿瘤标记物测定的临床意义

摘　　要：目的测定胃癌患者血清中肿瘤标记物MMP-9,MMP-2,CA72-4,P53-IgG,MG7-Ag的水平,以评价其在胃癌的诊断和预测病理分型和转移及预后的意义。方法用双抗体夹心法测定血浆MMP-9,用ELISA法测定血清MMP-2,CA72-4,P53-IgG,MG7-Ag。结果上述肿瘤标记物在非胃癌组和胃癌组中的表达水平有显著性差异(P<0.01)。若以高于非胃癌标记物平均数加2倍标准差作为诊断肿瘤的界限,则五种标记物联合检测的敏感性为96.8%,特异性为93.8%。但胃癌的浸润深度、淋巴结转移、远处转移对各标记物的表达水平无明显的影响(P>0.05)。结论采用多种肿瘤标记物联合检测对胃癌的诊断有较大的意义。Objective To determine serum MMP-9, MMP-2, CA72-4, P53-IgG and MG7-Ag and to evaluate their clinical signifieanees including diagnosis, prediction of pathological type, metastasis and prognosis in the process of gastric cancer. Methods Sandwich enzyme-linked immunosorbent assay was used to determine plasma MMP-9 and ELISA was used to determine sermn MMP-2, CA72-4, P53-IgG and MG7-Ag. Results The levels of the above 5 tumor markers significantly increased in the gastric cancer group compared with those in the normal group （P 〈 O. Ol ）. If twice standard deviation above average level of 5 tumor markers in the normal group was considered as the diagnosis criteria of gastric cancer, the examination＇ s sensitivity of the five tumor markers combination was 96.8% and the specificity vas 93.8%. But the expression of the five tumor markers was not related to invasive depth or lymph node metastasis or metastasis. Conclusion Examination of combined several tumor markers is of use for the gastric cancer diagnosis.
