喉部分切除术后喉功能重建的疗效观察

摘　　要：目的：探讨喉部分切除后通气发声功能重建的不同手术方法对其功能恢复的影响。方法：分析喉部分切除术108例（垂直半喉切除76例，包括术后声门Ⅰ期成形18例；水平半喉切除11例；声带切除15例；超半喉切除6例）的临床资料。术后随访3-5年，观察局部及功能恢复情况。结果：术后拔管97例，拔管时间8d至1年，未拔管11例（垂直半喉6例，水平半喉2例，超半喉3例）。全部病人均可发声或堵管发声，完成言语交流，其中术后Ⅰ期成形的患者发声质量最好，接近正常人。术后13例局部复发、咽后转移及颈部淋巴结转移。5例死亡。结论：根据肿瘤生长部位和范围正确选择不同的喉部分切除术，既可彻底切除肿瘤，又能比较好的保留喉呼吸和发声功能。Objective, To investigate the effects for reconstruction of laryngeal function of ventilation and vocalization with different operative methods. Method..176 cases of laryngocarcinoma were studied from 1990 to 1997. In the cases, 165 were male and the other were female,aged from 29 to 80 years old. Partial laryngectomy was performed in 108 cases ,including vertical half-laryngectomy（VHL, 76 cases, 18 with Ⅰ stage laryngoplasty）, horizontal half-laryngectomy （HHL, 11 cases） ,condectomy （15 cases）and over-half-laryngectomy（OHL, 6 cases）. Follow- up surveys were made after operation to investigate local and functional reduction. Results ：The number of the cases of decannulation was 97, from 8 days to 1 year after operation, 11 cases without decannulatin（VHL 6,HHL 2 and OHL 3）. All the patients could speak after dacannulation or in obstruction of canals, the best state in . 13 cases had local or retropharynx recurrence and lymphatic metastasis in neck ,and 5 patients died. Conclusion： In order to resect tumors completely and keep laryngeal function,it is necessary to select different operations according to the site and range of tumor growth.
