出院病人随访管理系统构建和实践

摘　　要：出院病人随访管理系统的意义就是利用医院信息管理资源,高效、便捷地访问出院病人。与以往随访的最大区别就是采用了数据库挖掘技术、网络技术、通讯技术,对所有出院病人进行跟踪随访和管理,跟踪建立病人随访健康档案,并联系医生指导出院病人康复。出院病人随访管理系统的应用进一步加强了医生与出院病人的沟通渠道,为医疗机构开辟了一种新的服务于病人的方式和服务渠道。我院2005年初正式使用以来,对95%以上出院病人进行跟踪随访,取得了良好的社会效益和口碑。这是国内医院尝试通过信息技术平台建立出院病人随访健康档案和医生指导的新模式。The importance of following-visiting management system is to visit patients ieaving the hospital efficiently and conveniently with hospital information management resource. The difference from traditional method is that the system taking use of data technology, net technology, communication technology follow all the patients who have already left the hospital and establish health file for them and connect the doctor to further help the patients to recover. The application of this system enhances the communication channel between doctors and patients having left the hospital, and increase a new service channel and method for hospitals to serve its patients. Since its formal use in 2005, the system has made following visit for 95% patients who have left hospitals and gained good social benefit and reputation. This is a new model for domestic hospitals to set up health file for patients through information technology platform and for doctor＇ s guide.
