摘 要:目的:进一步探究脑电图(EEG)在脑梗死急性期辅助诊断和判断预后方面的应用价值。方法:16例脑梗死急性期患者,头颅CT检查正常,发病24~48 h内进行床旁脑电描记,选择理想信号段进行功率谱分析,计算各频段功率值及指数。发病后1~4周行头颅MRI检查以明确责任病灶,并随访病情变化。结果:脑电图异常率为88%,功率谱分析显示患侧α频段功率值降低,慢波频段功率值增高,α指数显著下降,δ指数显著升高。脑电图定侧与头颅MRI基本相符。结论:脑电图反映脑功能活动,对皮质缺血十分敏感,并有助于判断预后。计算机分析方法可提供更多信息。脑电图操作方便,在脑梗死早期诊断中是较有价值的检查方法。Objective:To explore the value of EEG in the assistant diagnosis and prognostication of the acute cerebral infarction. Methods:Bedside electroencephalograph was examined in 16 patients with acute cerebral infarction by 24-48 hours whose cranial CT scan is normal. The selected data was analysed to get the power spectrum and the indexes of α、β、θ、δ waves. Results:In the patients,the abnormity rate of EEG is 88% , power spectrum reveals that α power values of the lesion lateral decrease and slow wave power values increase. In the lesion lateral α index significantly decreases, while δ index increases. The diagnostic infarction lateral based on EEG is consistent with cranial MRI. Conclusion: EEG reveal the function of the brain, which is very sensitive to the cortex ischemia and contribute to prognostication. Computer analysis of EEG provide more information. The practice of EEC- is simple, and it is valuable in the assistant diagnosis of the acute cerebral infarction.
