胃癌的超声内镜分期和基质金属蛋白酶-7表达的相关性

摘　　要：目的研究胃癌的超声内镜分期与基质金属蛋白酶-7表达的相关性。方法33例早期胃癌和56例进展期胃癌术前行超声内镜检查、早期胃癌部分行EMR术,进展期胃癌行根治或姑息手术,所有病例均经病理证实;用免疫组织化学法检测MMP-7的表达。结果早期胃癌患者的癌组织MMP-7表达水平明显低于进展期胃的组织水平(P<0.02),且与胃癌的组织分型明显相关(P<0.01)。2例黏膜层癌MMP-7表达强阳性的病例,行部分胃切除根治术,有1例发现淋巴结转移,但未被超声内镜发现。4例黏膜下层癌MMP-7表达强阳性的病例,3例发现有淋巴结转移,超声内镜发现1例。结论胃癌的超声内镜分期与基质金属蛋白酶-7表达相关,MMP-7高度表达的早期胃癌患者应行根治手术。[Objective] The correlation of clinical study of the stage of gastric cancer of endoscopic ulirasonograghy with MMP-7 expresion was investigated. [Method] 33 patients with the early gastric cancer discovered by mucous stained and 56 patients with the anvanced gastric cancer, staged by Endoscopic ultrasonograghy （EUS） before endoscopic mucosal resection （EMR） or resection or partly palliative surgery, all cases confirmed finally by pathology. MMP-7 expression was eximined by Immunohischemistry. [Result] The level of MMP-7 expression in tissues of early gastric cancer identified by EUS is significantly lower than that of the advanced cancer （P 〈0.02） and that correlated with differeniated tissue type of gastric carcinoma （P 〈0.01）, the lymph node matastasis is found in one of two cases with strongly positive MMP-7 expression in issue of intramucosa cancer unidentified by EUS and three cases of submucosa cancer and one of them identified by EUS was comfirmed by pathology. [Conclusion] It is significant that correlation of clinical study of the stage of gastric cancer with endoscopic uhrasonograghy with MMP- 7 expression, the cases of strongly positive expression of MMP-7 in tissue of early gastric cancer may be resected.
