内镜超声检查术在胃间质瘤中的诊断价值

摘　　要：目的探讨内镜超声检查术(Endoscopicultrasonography,EUS)对胃间质瘤的诊断价值。方法对19例胃间质瘤患者进行EUS检查,并将检查结果与术后病理结果进行对照。结果胃间质瘤在EUS中表现为圆形或类圆形、均匀或不均匀的低回声,边界清晰。采用EUS诊断胃平滑肌瘤16例,胃囊肿2例,胃脂肪瘤1例。手术后病理诊断胃平滑肌瘤18例,胃平滑肌肉瘤1例。结论EUS对胃间质瘤诊断率为84.22%,EUS既能显示胃壁各层,又能反映肿瘤浸润胃壁的深度,因此EUS不仅可用于胃间质瘤的诊断,而且对手术方式的选择具有一定的指导意义。[Objective] To evaluate the significance of endoscpic ultrasonography （EUS） in the diagnosis of gastric stromal tumor. [Methods] 19 cases of gastric stromal tumor were diagnosed by EUS, the results were compared with the postoperative pathological findings. [Results] The ultrosonography of gastric stromal tumor manifested as lesions with hetero-and/or homo-geneous hypoechoic patterns and readily distinguished border. 16 cases of gastric leiomyoma, 2 cases of gastric cystis and 1 case of gastric adipoma were detected by EUS, while 18 cases of gastric stromal tumor and 1 case of gastric leiomyosareoma were diagnosed pathologically. [Conclusions] The coincidence with tumor invasion can be distinguished clearly by EUS, leading to definite diagnosis of gastric submucosal tumors, especially gastric stromal tumor.
