腹腔镜下胃癌根治术患者外周静脉血CK_(20) mRNA的检测及其临床意义

摘　　要：目的检测腹腔镜胃癌根治术患者外周静脉血CK20mRNA的表达,并探讨其临床意义。方法应用RT-PCR方法检测44例行腹腔镜辅助下胃癌根治术和29例开腹胃癌根治术患者外周静脉血的CK20mRNA。结果TNM分期Ⅲ期胃癌患者外周静脉血的CK20mRNA显著高于Ⅰ期、Ⅱ期患者(P<0.05)。腹腔镜辅助下胃癌根治术患者CK20手术开始时阳性率为22.7%(10/44),手术结束时阳性率为27.3%(12/44),两组之间无显著性差异(P>0.05),手术前后和开腹组无显著差异(P>0.05)。结论胃癌患者外周静脉血CK20mRNA的水平和TNM分期有关,腹腔镜胃癌根治术并不增加外周静脉血CK20mRNA的阳性率。Objective To detect CK20 mRNA expression in peripheral blood in patients underwent laparoscopic radical gastrectomy and to discuss its clinical significance. Methods CK20 mRNA was detected in peripheral blood of 44 gastric cancer patients underwent laparoscopic radical gastrectomy and 29 patients with open radical gastrectomy by RT-PCR. Results CK20 expression was significantly higher in the patients categorized to phase Ⅲ than phase Ⅰ / Ⅱ according to TNM staging （P 〈 0.05）. The preoperative positive rate of CK20 mRNA was 22.7% （10/44）, while the postoperative positive rate was 27.3% （12/44） in patients underwent laparoscopic radical gastrectomy, and there was no significant difference between the two groups （ P 〉 0.05 ）. The positive rate of CK20 mRNA expression had no significant difference between the laparoscopic and open radical gastrectomy either preoperatively or postoperatively. Conclusion The amount of cancer cells in peripheral blood of gastric cancer patients concern with TNM staging. Laparoscopic radical gastrectomy does not increase the positive rate of cancer cells in peripheral blood.
