OUR-XGD伽玛刀分次治疗和外照射在鼻咽癌中的临床应用

摘　　要：目的 探讨OUR-XGD旋转式伽玛刀分次治疗和外照射在鼻咽癌治疗中的作用，以及减少并发症的发生和提高生存质量。方法 38例鼻咽癌病人应用旋转式伽玛刀进行分次治疗，总剂量45Gy，覆盖原发病灶边缘0．5～1．0cm，等剂量曲线40％～50％。治疗后1周内根据肿瘤分期、转移程度和不同部位选用不同剂量进行补充外照射放疗。结果 32例获随访，其中3年以上28例，5年以上4例，失访6例，无复发与死亡病例，术后并发症5例（15．6％）。结论 OUR-XGD伽玛刀分次治疗加外照射是又一种可选择的治疗手段，但其治疗效果、治疗方法仍有待于进一步临床观察、探讨和提高。Objective To study the effects of OUR-XGD gamma knife time-dividing treatment combined with outside irradiation for treatment of nasopharyngeal carcinoma （NPC）. Methods Thirty-eight patients suffering from NPC were treated by OUR-XGD gamma knife time-dividing treatment with total dosage of 45 Gy and equaldosage curve of 40%～50%. The edge area of 0. 5- 1.0 cm around primary focus was covered. One week after gamma knife time-dividing treatment, outside irradiation was applied to the patients according to the clinical stage, metastasis and location of tumor to complete the treatment. Results Of all cases, neither occurrence nor death occurred after follow-up for 36 to 60 months but postoperative complications occurred in 5 cases （15.6%）. Conclusion OUR-XGD gamma knife time-dividing treatment combined with outside irradiation is another therapeutic choice for NPC with the effect and the method still being further investigated.
