早期胃癌直径、淋巴结微转移与组织MMP-7表达强度的相关性

摘　　要：目的:研究早期胃癌直径、淋巴结微转移与基质金属蛋白酶-7表达的相关性。方法:55例早期胃癌经黏膜颜色、术后病理证实为早期胃癌。用免疫组织化学法检测MMP-7的表达强度和淋巴结微转移。结果:直径大于20mm早期胃癌的MMP-7表达阳性率明显高于直径小于10mm的病例(P<0.045),亦高于直径10～20mm病例的表达,但无统计学意义(P>0.104)。MMP-7的表达与胃癌的组织分型明显相关,低分化癌MMP-7的表达高于高分化癌(P<0.015),亦高于中分化癌的表达,但无统计学意义。直径大于20mm早期胃癌的淋巴结转移率高于直径10～20mm的淋巴结转移率。结论:早期胃癌直径、淋巴结微转移与基质金属蛋白酶-7的表达相关,直径大于20mm患者MMP-7表达强阳性率和淋巴结转移率高。Objective To investigate the correlation of the cancerous diameter of early gastric cancer and lymph node micrometastasis to matrix metalloproteinase-7 （MMP-7） expression in tissue in patients with early gastric cancer Methods Fifty five patients with early gastric cancer were discovered by mucosa staining, confirmed finally by pathology. Expression of MMP-7 in tissues and lymph node micrometastasis was determined by immunohistochemical assay. Results The positive rate of MMP-7 expression of cancerous diameter larger than 20 mm in tissues in patients with early gastric carcinoma was significantly higher than that of cancerous diameter smaller than 10 mm in patients with early gastric cancer（ P 〈 0.045） and higher than that of the cancerous diameter in between 10 mm and 20 mm, but with no statistical significance（ P 〈 0.104）. The expression of MMP-7 in tissues was associated with differentiated tissue type of early gastric carcinoma. The expression in the low differentiated group was higher than that in the high differentiated group （ P 〈 0.015 ）, and higher than in the moderate differentiated group, but with no statistical significance（ P 〈 0. 167）. The incidence of lymph node micrometastasis in patients with early gastric carcinoma larger than 20 mm in diameter was higher than that of 10 mm-21 mm. Conclusion The cancerous diameter of early gastric cancer is significantly related to MMP-7 expression, the cases of strongly positive overexpression of MMP-7 in tissue in patients with early gastric cancer are associated more likely with lymph node micrometastasis.
