原发性肝癌介入治疗前后血清前白蛋白测定的价值

摘　　要：目的:探讨血清前白蛋白(PAB)检测在原发性肝癌介入治疗围手术期肝功能储备及预后评价的价值。方法:分析90例接受动脉介入化疗栓塞的原发性肝癌患者围手术期血清PAB水平与肝功能损伤程度、介入治疗方式及术后肝功能衰竭的关系。结果:90例肝癌患者术前血清PAB值明显低于正常对照组(P<0.01),且患者术前血清PAB值有显著差异(P<0.01),为Child A>Child B>Child C,血清PAB值与肝功能Child分级一致。对于肝功能Child B和Child C级患者,不同的介入治疗方法术后血清PAB值有显著性差异(P<0.01)。结论:血清PAB值可较为敏感而准确地反映介入治疗围手术期的肝脏储备能力及肝功能损害程度,观察血清PAB值变化可及时发现肝功能衰竭,并为评价疗效及判断预后提供依据。Objective： To investigate the clinical value of peri-interventional operative prealbumin （PAB） determination in patients with primary liver cancer. Methods：The PAB was examined peri-operatively in 90 patients with primary liver cancer to test the correlation between PAB and liver function,the degree of hepatocirrhosis,interventional therapy method and hepatic failure. Results： The preoperative mean value of prealbumin was significantly lower as compared with that of the control group （P〈0.01） ;the preoperative values of PAB in different Child grading patients with primary liver cancer were different significanly （P〈0.01）,Child A〉Child B〉Child C,the PAB value corresponded with Child grading of the liver function and the cirrhotic degree of liver. In the patients with liver function of Child B and C,the postoperative mean value of PAB in different interventional therapy methods were different significantly （P〈0.01）. Conclusion： The value of PAB can sensitively and accurately reflect liver reserve ability and damage degree of peri-interventional operative liver function. Hepatic failure can be found in time and the prognosis of the patients with primary liver cancer can be predicted by testing the value of PAB continually.
