副肿瘤性天疱疮的超声及CT特征

摘　　要：目的探讨副肿瘤性天疱疮(PNP)的超声及CT特征与临床表现。方法回顾性分析手术病理证实的8例副肿瘤性天疱疮患者完整的临床、实验室、影像学、病理学及术后随访资料。结果8例副肿瘤性天疱疮主要临床特征和超声、CT表现综合归纳为:多形性皮肤损害伴疼痛性黏膜糜烂溃疡,同时合并内脏软组织肿块。肿瘤部位:腹膜后6例,胸腔纵隔2例。病理类型:巨淋巴结增生7例,非霍奇金淋巴瘤1例。结论超声及CT可以发现副肿瘤性天疱疮时皮肤黏膜损害出现之前的内脏肿瘤,且应首选腹部超声检查,其次是胸部CT。Objective To investigate the clinical feature as well as ultrasound and CT characteristics of paraneoplastic pemphigus（PNP）. Methods Total datum of the clinical, laboratory, imaginology, histopathology and the follow-up observations of 8 PNP cases those were finally confirmed by operation were analyzed retrospectively. Results The clinical characteristics as well as ultrasound and CT findings of the 8 PNP cases were summaried as follows： polymorphous cutaneous impairments and painful mucous erosion and ulceration were all companied by visceral neoplasms with 6 of which arise from retroperitoneum and 2 from mediastinum. Histopathologic classifications were 7 Castleman＇s disease and 1 non-Hodgkin＇s lymphoma. Conclusions Vesceral neoplasm often occurs before mucocutaneous impairments in PNP patients, as one of diagnostic procedures, abdominal sonography is the first choice, the second is chest CT.
