美沙酮与丁丙诺啡对海洛因依赖者脱瘾治疗效果比较

摘　　要：目的对比研究美沙酮口服液和丁丙诺啡对海洛因依赖者替代疗效,评价脱瘾效果。方法采用随机分组将120病例分为美沙酮组80人和丁丙诺啡组40人,两组分别设置递减法给药方案,单盲法进行1疗程10次脱瘾治疗,以自编21项“戒断症状观察积分表”(ows)记录症状变化,以t检验比较2组OWS得分。结果美沙酮治疗组OWS得分(7.01±6.26～21.40±10.57)比丁丙诺啡组得分(12.25±10.26～32.79±11.97)变化小;在治疗初期(第2、3次给药)得分低于丁丙诺啡组,P<0.01,有极显著性差异;治疗后期(第9次给药)得分也低于丁丙诺啡组,P<0.05,有显著性差异;治疗中期(第4～8次给药)2组OWS积分差异则无显著性差异,p>0.05;此外2组在OWS量表中主要症状出现率也有一定差异。结论美沙酮口服液替代法在剂量调整,快速缓解戒断症状,停药反应等方面优于丁丙诺啡替代法。Objective To compare the curative effect on human of heroin dependence between Methadone and Buprenorphine. Methods A randomized single blindness control Case trail was made , 120 cases were divided to 2 groups in randomization ： there are 80 cases in Methadone Group （case group） and 40 cases in Buprenorphin Group（control group）. Drug administration methods of down regulation step by step were made in different groups .1 course of treatment of single blindness was made in 2 groups.＂To observe withdrawal symptoms intergral scale＂： （OWS） was adopted , Student t-test was used to compare the score of OWS of 2 groups. Results： The scores of OWS of case group（7. 01 ±26-21.40 ±0.57）changed smoothly compare with that of control group （12.25 ±0.26-32.79 ±1.97）： in initial stage treatment （2^nd 3^rd times administration）, the score of case group was lower than that of control, P〈0.01; in late stage of treatment （9^th times administration）, the score was lower than that of control , P〈0. 05; in medium stage of treatment ,there had no significance difference in the 2 groups about the scores of OWS , P〉0. 05; in addition ,the appearance rate of primary symptom of OWS of 2 groups had some differences .Conclusion The substitution treatment of methadone is better than the treatment of Buprenorphine in adjustment of dosage, prompt lysis withdrawal symptoms and the reaction of suspend drug .
