大肠镜诊断治疗11876例次回顾研究

摘　　要：目的通过大样本病例探讨安徽江淮地区近15年来大肠疾病构成情况和特征。方法回顾分析总结1989年4月至2004年5月15年间11 876例次的大肠镜诊断治疗资料。结果共完成大肠镜检查10 878例,治疗性大肠镜998例次。检出大肠和回肠末端疾病20余种,检出率为63.0%。其中大肠非特异性炎2 643例(24.3%),大肠息肉1 392例(12.8%)、溃疡性结肠炎(UC)968例(8.9%)、大肠癌782例(7.2%)、回肠末端病变261例(2.4%)、血管畸形120例(1.1%)、其他少见、疑难病309例(2.8%)。未见异常4 024例。在1 392例各种息肉中,有Ⅱ-Ⅲ级不典型增生79例(5.7%),息肉癌变102例(7.3%)。采用高频电凝电切摘除息肉1 327颗/998例次,其中腺瘤性息肉547例/751颗,全瘤病理为Ⅰ型早期癌14例,早期癌检出率1.8%。结论大肠息肉、溃疡性结肠炎及大肠癌已成为大肠的主要疾病,对有下消化道症状者应常规行大肠镜检查。Objective To explore the characteristics of colorectal diseases construct by a large amour of cases during the last 15 years in An hui jianghuai area. Methods Total 11876 case who had colonoscopy performed in our hospital from April 1989 to May 2004 were retrospectively analyzed. Results More than 20 kinds of different colorectal diseases and terminal ileal diseases were detected, and detective rate of diseases was 63%. Colitis was diagnosed in 2643（24.3% ） ,1392 （ 12.8% ） patients with colorectal polyps were found, 968 （8.9%）with ulcerative colitis （ U C） ,782 （ 7.2% ） with colorectal cancer,261 （ 2.4% ） with terminal ileal diseases and 120 （ 1.1% ） with vascular malformation, the remainder having rare or difficult diseases were 309 （2.8%）. Colonoscopy showed normal in 4024 cases. Of the 1392 patients with polyps,79 （5.7%）cases with moderate to severe atypical hyperplasia and 102 （7.3%）with canceration, 1327 polyps among 998 cases were removed with high frequence electricied current under the endscopy,and early cancer in 14 （ 1.8% ）. Conclusion There has been an increasing number of colorectal diseases, mostly with Colorectal polyps,ulcerative colitis, and colorectal cancer. Colonoscopy can be used as a routine examination for patients complaining of lower digestive tract symptoms.
