摘　　要：目的:研究非酒精性脂肪肝(NAFL)患者血清铁、转铁蛋白、血清铁蛋白的水平,确定诊断指标。方法:分别用原子吸收光谱法和放射免疫法检测患者和正常对照者血清铁、转铁蛋白、血清铁蛋白水平。采用1sec快速肝穿刺法,对68例NAFL患者取肝组织标本,行HE和铁染色。结果:患者组血清铁、转铁蛋白、血清铁蛋白水平分别为19.5±11.8μmol/L、44.2±10.3μmol/L、178.7±72.1ng/mL,对照组20例血清铁、转铁蛋白、血清铁蛋白分别为10.5±5.7μmol/L、40.7±10.3μmol/L、139.3±71.9ng/mL。患者组血清铁及血清铁蛋白与对照组有显著性差异(P<0.01)。重度NAFL患者转铁蛋白与对照组间比较有统计学差异(P<0.05)。结论:NAFL患者多出现血清铁、转铁蛋白及血清铁蛋白增高,这三项指标可作为诊断NAFL的重要参考指标。Objective： To determine the levels of serum iron, transferrin, and serum ferritin in nonalcoholic fatty liver （NAFL） patients and find the diagnostic indicator. Methods： The levels of serum iron ,transferrin, and serum ferritin were detected by atomic absorption spectrum method and radioimmunoassay. Totally 68 NAFL patients were. involved in this study. The liver specimens were obtained by one second liver biopsy and stained by hematoxylin - eosin and Perl ＇ s Prussian blue. Results： The level of serum iron ,transferrin, and serum ferritin in 68 NFAL patients were 19.5 ± 11.8 μmol/L, 44.2 ± 10.3 μmol/L, and 178.7 ±72.1 ng/mL, respectively. In normal control group the level of serum iron, transferrin, and serum ferritin were 10.5 ±5.7 μmol/L, 40.7 ± 10.3 μmol/L and 139.3 ± 71.9 ng/mL, respectively. The difference in serum iron and serum ferritin was significantly different between NFAL patients and normal control （ P 〈 0.01 ）. The transferrin level in patients with severe NAFL was significantly different compared with normal control （ P 〈 0.05 ）. Conclusion： The levels of serum iron, transferrin, and serum ferritin were higher in NAFL patients. These three indices could be the important markers for diagnosis of NAFL.
