重组IL-11治疗化疗后所致血小板低下的临床观察

摘　　要：目的观察重组人白介素-11对化疗后所致血小板低下的治疗作用和不良反应。方法化疗后血小板低于50×109/L的恶性肿瘤患者42例,随机分为两组,治疗组用IL-11 25μg/(kg.d)皮下注射,连用7 d;对照组皮下注射生理盐水,以用药1周和14 d后外周血血小板的值进行对照。结果用药1周后治疗组与对照组血小板的均值分别为(91.26±21.54)×109/L和(70.34±19.72)×109/L,14 d后血小板的均值分别为(145.67±88.92)×109/L和(87.39±13.56)×109/L,两者间差异有显著性(P<0.05)。治疗组血小板>100×109/L者16例(76.2%),对照组3例(14.3%,P<0.01)。治疗组1例患者输注血小板,对照组3例患者输注血小板。主要不良反应为关节肌肉酸痛、乏力、水肿等,其他不良反应少见。结论重组人白细胞介素-11可治疗化疗后血小板低下,缩短血小板下降的持续时间,减少血小板的输注次数,而且不良反应可以耐受,值得临床进一步观察。Objective To observe the efficacy and toxicity of rhIL-11 in treatment of chemotherapyinduced thmmbocytopenia. Methods Forty-two patients whose platelet count was less than 50× 10^9/L after chemotherapy were divided randomly into two groups. IL-11 was administered subcutaneously [ 25μg/（ kg · d） ]for 7 days in treatment group. The control group was administered with NS. The platelet counts were compared after 1 week and 14 days of treatment. Results After 1 week and 14 days of treatment, mean platelet count of the treatment group was（91.26 ± 21.54） × 10^9/L and （ 145.67 ± 88.92） × 10^9/L respectively,while that of the control group was（70.34 ± 19.72） × 10^9/L and （87.39 ± 13： 56） × 10^9/L respectively（P 〈 0.05 ）. Two weeks later, there were 16 （76.2%） and 3 （ 14.3% ） patients whose platelet count was higher than 100 × 10^9/L in the treatment group and the control group respectively（P 〈0.01 ）. One patient and 3 patients respectively in the two groups needed platelet transfusion. Major adverse effects were myalgia and arthralgia, fatigue, edema, etc. Conclusion rhIL-11 can improve thrombocytopenia induced by chemotherapy and decrease the need for platelet transfusion. Its toxicity is tolerable. It is worthy of further observahon.
