肝癌多支供血及栓塞后侧支供血的研究

摘　　要：目的:通过本研究探讨多支供血和栓塞后侧支供血在肝癌动脉化疗栓塞(TACE)治疗中的意义.材料与方法:分析100例肝癌动脉造影表现,探讨肝癌多支供血和肝动脉栓塞后肝内外侧支供血的情况及与疗效的关系.结果:肝外供血动脉和栓塞后肝内外侧支供血动脉的出现与肿瘤的部位相关;单一动脉供血和无侧支供血者疗效好,多支供血和侧支供血者疗效差.结论:多支供血和肝动脉栓塞后侧支供血是影响疗效的重要因素,指出在TACE 时对肝外供血动脉和肝外侧支供血动脉应作永久性栓塞.To discuss the significance of multiple feeding vessels and collateral blood sup- ply after TACE in hepatic carcinoma,we observed the findings of hepatic arteriography in 100 pa- tients with hepatic carcinoma,analyzed the relationship between the rapeutic effect and multiple feeding vessels and collateral blood supply after TACE.Close relation between the occurenec of extrahepatic blood supply or collateral blood sullpy after TACE and the tumor location was found,satisfactory effects were observed among the cases with single arterial hlodd supply and no collateral bloosd supply comparing with those having multiple feeding vessels and collateral blood supply.It is indicated that the permanent embolization of extrahepatic feeding vessels should be performed when extrahepatic blood supplies occur when embolization therapy.
