摘 要:背景:有研究证实,稀释胃液固有荧光光谱可用于胃癌临床诊断和筛查,但检测条件不同时,其结果不尽相同。目的:确定胃液固有荧光光谱的最佳检测条件。方法:随机收集162例胃部良、恶性病变患者(其中恶性病变者54例)的胃液,用pH7磷酸盐缓冲液分别以1∶5、1∶10、1∶20、1∶40、1∶80稀释;继而分别以pH3、5、7、8、9、10、11磷酸盐稀释溶液和超纯水稀释20倍,分别保存于室温、4℃、-20℃、-40℃、-80℃和液氮中。应用RF-5000型荧光分光光度计检测。结果:良、恶性病变患者的胃液荧光光谱均有两个发射峰,分别位于330~360nm和579.2nm处。1∶20稀释时荧光光谱第一峰荧光强度(P1FI)较强,良、恶性病变之间的P1FI差异较大,受试者工作特征(ROC)曲线下面积为0.87;pH11时,良、恶性病变之间的P1FI差异最大,ROC曲线下面积最大(0.82)。液氮保存对P1FI的影响最小,保存1个月不影响敏感性。结论:稀释浓度为1∶20、磷酸盐稀释溶液pH11、液氮保存不超过1个月时,胃液固有荧光光谱检测结果较好。本研究结果对规范胃液固有荧光光谱的检测实验条件、提高检测的稳定性和保证结果的可靠性有重要意义。It has been demonstrated that determination of gastric juice autofluorescence spectrum is valuable in the detection and screening of gastric cancer. However, the results were variable when the testing parameters were different. Aims: To determine the best testing parameters of gastric juice autofluorescence spectrum. Methods: A total of 162 patients including 54 with malignant lesions were enrolled. Gastric juice from all patients was diluted to 1:5, 1:10, 1:20, 1:40, 1:80 with pH 7 phosphate buffers. Then gastric juice was again diluted to 1:20 with pH 3, 5, 7, 8, 9, 10, 11 phosphate buffers, respectively. Before testing, gastric juice was stored at room temperature, 4 ℃, -20 ℃, -40 ℃, -80 ℃ and in liquid nitrogen, respectively. The autofluorescence spectrum was determined with RF-5000 fluorescence spectrophotometer. Results: There were two peaks of autofluorescence spectrum in all patients, one was maxinal fluorescence intensity at the range from 330-360 nm, another at 579.2 nm 1:20 was the best dilution because the fluorescence intensity of first peak (P1FI) was high, the difference of P1FI between malignant and benign lesions was more significant, and the area under reeeiver operating characteristic (ROC) curve was 0.87; pH 11 was the best pH of the diluted solution because the difference of P1FI between malignant and benign lesions was most distinct and the area under ROC was 0.82. Storage of gastric juice sample in liquid nitrogen had the least influence on P1FI and storage less than 1 month had no influence on its sensitivity in detecting and screening malignant gastric lesions. Conclusions: Our results have shown that the best testing parameters are 1:20 dilution with pH 11 phosphate buffer, and the gastric: juice sample should be stored in liquid nitrogen less than 1 month.
