超声引导下射频消融治疗肝癌的价值探讨

摘　　要：目的评价超声引导射频消融对肝脏恶性肿瘤的治疗效果,探讨其临床使用价值。方法30例患者共49个病灶接受了治疗。治疗后观察肿瘤消融情况,比较治疗前后瘤体内血供情况、肿瘤大小和甲胎蛋白(AFP)值的变化以及并发症。结果49个病灶全部消融成功。在3～14个月的随访中,共有9例患者复发,均再次进行了射频消融治疗。2例死于肝功能衰竭。结论超声引导射频消融是一种安全有效的经皮热消融技术。Objective To evaluate the efficacy of ultrasound-guided radiofrequency thermal ablation in treatment of hepatic carcinoma. Methods Thirty hepatic caner patients with 49 malignant lesions underwent radiofrequency thermal ablation. The ablation areas were observed and the vascular flow to tumor nodules, radiographic images, tumor size, clinical findings and AFP value were assessed before and after the radiofrequency thermal treatment. Results Forty-nine nodules were successfully ablated with radiofrequency thermal treatment. During follow-up of 3-14 months, 9 of 30 patients had recurrences and all received the second radiofrequency thermal ablation successfully. Two among 30 patients died from hepatic failure. Conclusion Pereutane ous radiofrequency ablation is a safe and effective technique for treatment of liver tumors.
