不同角膜缘状态翼状胬肉的手术疗效观察

摘　　要：目的 观察不同角膜缘状态的翼状胬肉用羊膜移植联合自体角膜缘移植术治疗的效果。方法 将251眼翼状胬肉患者按角膜缘状态不同分组，采用羊膜移植联合自体角膜缘移植术治疗，定期随访，裂隙灯检查、照相。结果 术后随访3～46个月，眼表愈合迅速，视力有所提高，总的术后复发率较低（仅5．58％）。上、下角膜缘有正常血管分布和Vogt栅的患眼组尚未观察到复发；而角膜缘血管异常且无Vogt栅结构的患眼组术后复发率为14．29％．两组差异有统计学意义（P〈0．01）。结论 翼状胬肉采用羊膜移植联合自体角膜缘移植术治疗，复发率较低；患眼角膜缘状态对判断手术预后十分有价值。Objective To observe the results of pterygium with different limbal condition using amniotic membrane transplanatation and limbal autograft transplantation method. Methods Two hundred and fifty one eyes of pterygium were divided into different groups according to the different limbal condition treated by pterygia excision with amniotic membrane transplanatation and limbal autograft transplantation, checked up regularly, observed with slit lamp microscope and photo taking. Resuits Two hundred and fifty one eyes with follow-up of 3- 46 months had shown improved visual acuity, rapid surface healing and decreased recurrence rate. The total recurrence rate was only 5. 58 %. There was non recurrence cases were observed the group of limbal condition with normal palisades of Vogt and vessel net. The recurrence rate of limbal condition with irregular palisades of Vogt and vessel net was 14.29 %. Conclusion The pterygium with ammiotic membrane transplanatation and limbal autograft transplantation has less recurrence rate, different limbal condition is very useful injudging the prognosis.
