自制碘-125支架治疗进展期食管癌的临床应用研究

摘　　要：目的探讨碘-125粒子支架治疗食管癌的疗效及碘-125粒子治疗剂量。方法75例患者分为治疗组(A组)45例,对照组(B组)30例,A组将0.4m c i、0.6m c i、0.8m c i三种不同剂量碘-125粒子捆绑在带膜支架上,分别于3月、6月、12月、15月观察总并发症发生情况、肿瘤生长转移情况、总生存率、不同剂量作用下生存率。结果A组在3月、6月、12月肿瘤生长阻塞支架发生率显著低于B组,新增转移癌灶低于B组,平均生存时间明显长于B组,p<0.05;0.6m c i、0.8m c i两种剂量组在阻止肿瘤生长、转移、延长生存率等方面优于0.4m c i剂量组,0.6m c i、0.8m c i两种剂量组间无统计学差异。结论碘-125粒子支架治疗食管癌可显著改善吞咽困难,有效阻止肿瘤生长、转移,延长生存期,碘-125粒子剂量以0.6m c i为宜。Objective To study the effect of attaching 125-idoine seeds with self-expanding metal stents to treat progressive esophageal cancer and its dosage. Methods 75 cases were divided into two groups, the treating group and the control group. The patients of group A were implanted self-expanding metal stents with 125-idoine seeds while those in group B were implanted self-expanding metal stents. The dose of 125-idoine seeds was 0.4mci, 0.6mci, 0.8mci respectively in group A. The complications, the growths and metastasis of esophageal cancer, the total survival rate, the effect of different doses of 125-idoine seeds were investigated. Results The rate of cancer growth to obstruction stents was lower than that of group B in the third month, the sixth month, and the twelfth month respectively. New increasing cancer metastasis focus was lower in group A than that of group B in the twelfth month. The mean survival time 8.4 4. 3.5 months in group A was much longer than those 4.4 4. 2.6 months in group B. The dose of 0.6mci,0.8mci was prior to the dose of 0.4mci in blocking the growth of esophageal cancer and the metastasis , prolonging the survival time. It is no difference in 0.6mci group and 0. 8mci group. Conclusion The treatment of esophageal cancer by self-expanding metal stents with 125-idoine seeds can improve dysphagia, block the growth and metastasis of esophageal cancer, and prolong obviously the survival time. It is suitable to choose 0.6mci 125-idoine seed attaching self-expanding metal stents to treat esophageal cancer.
