摘　　要：目的:研究89例食管癌患者外周血中CK20、CK19及CEA mRNA的表达,并探讨其临床病理意义。方法:巢式RT-PCR技术检测89例食管癌患者术前外周血中CK20、CK19、CEA mRNA的表达。结果:89例食管癌患者外周血中CK20、CK19及CEA的阳性表达率分别为49.44%、34.83%及52.81%;三基因同时检测时,至少1个基因检测结果为阳性的表达率为71.91%,与食管癌的淋巴结转移和(或)远处转移具有相关性。结论:CK20、CEA可作为检测食管癌患者外周血微转移的生物学标志,RT-PCR联合检测多种标志物可为了解肿瘤的恶性生物学行为、指导临床治疗方案的选择、判断预后及监测疗效提供依据。Objective To investigate the expression of CK20,CK19 and CEA mRNA in peripheral blood from patients with esophageal cancer and to evaluate its clinicopathologic significance. Methods A total of 89 preoperative peripheral blood specimens obtained from patients with esophageal cancer were analyzed by CK20,CK19 and CEA-specific nested reverse transcriptase-polymerase chain reaction. Results Among 89 blood samples from patients with esophageal cancer, the expression of CK20, C K19 and CEA mRNA was 49.44% ,34.83 % and 52.81% , respectively. 71.91% were positive for at least one markers for these three genes, and were significantly correlated with the lymph node metastasis and（or） distant metastasis of the cancer. Conclusion The identification of circulating tumor cells with markers of CK20 mRNA and CEA mRNA in peripheral blood from patients with esophageal cancer using RT-PCR is feasible,and the molecular detection of circulating tumor cells by the use of multiple markers could help increase PCR positivity and sensitivity, and may be helpful for the selection of clinical managements and the judgment of prognosis and the surveillance of curative effect.
