骶骨肿瘤术后常见并发症的临床分析与防治

摘　　要：目的探讨常见的骶骨肿瘤术后并发症类型及相应预防治疗措施,为提高骶骨肿瘤的手术治疗水平、降低术后并发症的发生率提供依据。方法对1998年1月至2005年8月进行手术切除的92例骶骨肿瘤患者进行回顾性研究。探讨骶骨肿瘤术后常见并发症的类型、原因及相应处理措施。结果骶骨肿瘤患者围手术期内最常见的并发症为术中大量出血和伤口相关并发症:本组中14例(15.2%)围手术期失血量大于3000ml,切口局部发生延迟愈合的8例(11.59%),切口感染5例(7.25%)。在获得长期随访的69例患者中30例(43.48%)发生肢体麻木、括约肌功能障碍和男性性功能障碍。行ISOLA内固定者术后6例(17.14%)出现腰部僵直。结论术中大量出血、神经根丛损伤、伤口并发症、腰椎骨盆失稳-重建相关并发症是除肿瘤局部复发之外最常见骶骨肿瘤术后并发症类型,相应的预防治疗措施可以将并发症的危害降到最低。Objective This is a retrospective study of 92 cases of sacral tumors and the most common post-operative complications of sacral tumors and relative prevention and treatment strategy were discussed in order to improve the operation and to prevent the relative complications. Methods Records of 92 patients with sacral tumors who underwent operations were retrospective studied to review the most common postoperative complication types of sacral tumors, causes and relative treatments. Results Large amount of blood loss and wound complications were the most common complications of peri-operation stage of the sacral tumors. The blood loss during the operation over 3000ml were in 14 cases（15.2%） and delayed union of the incision occurred in 8（ 11.59%） while 5（7.25%） developed infection. In 69 cases who were followed-up, there are 30（43.48%） cases who developed limb paralysis and sphincter disorder and male sexual disfunction. 6 cases （17.14%） who underwent internal fixation developed the back stiffness. Conclusions Loss of large amounts of blood during the operation, neurological dysfunction, wound complications and the lumbopelvic instability complications are the most common complications in the surgical treatment of sacral tumors except local recurrence. Corresponding measures of prevention and treatment are efficient.
