KTP激光泪道疏通术后泪道扩张冲洗频率对其疗效的影响

摘　　要：目的探讨KTP激光泪道疏通术后不同的泪道扩张冲洗频率对其疗效的影响。方法将72例(80眼)单纯泪小(总)管阻塞和74例(78眼)鼻泪管阻塞患者分别随机分为A、B2组和C、D2组,其中A组38眼、B组42眼为泪小(总)管阻塞;C组38眼、D组40眼为鼻泪管阻塞;A、C2组术后连续3d每日扩张冲洗泪道1次,然后隔日1次至术后1周,以后每周2次至术后1个月;B、D2组术后第3d及术后1周各扩张冲洗泪道1次,以后每周1次至术后1个月,分别比较A、B2组和C、D2组的疗效。结果术后1个月,A、B2组的治愈率分别是84.2%,78.6%;C、D2组的治愈率分别是65.6%,62.5%,A、B2组相比和C、D2组相比,其治愈率均无明显差异(P>0.05)。术后3个月,A、B2组的治愈率分别是57.9%,83.3%;C、D2组的治愈率分别是47.4%,72·5%,A、B2组相比和C、D2组相比,其治愈率均有显著差异(P<0.05)。结论KTP激光泪道疏通术后,适当的泪道扩张冲洗次数可提高治愈率,而过度频繁的泪道扩张冲洗次数容易导致泪道粘膜瘢痕增生,降低治愈率。Objective To study the curative effect of different frequence of irrigating and dilating lacrimal duct after endolacrimal recanalisation surgery by KTP laser. Methods Seventy-two （80 eyes） cases of canaliculus obstruction and 74 cases （78 eyes） of nasolacrtmal duct obstruction, which received endolacrimal recanalisation surgery by KTP laser,were randomly divided into two groups A,B and C,D respectively. Group A, C received frequently irrigating and dilating lacrimal duct （once a day in the first 3 consecutive postoperative days, then once on alternate days for 4 days, followed by twice a week till a month）and group B, D received non-frequently irrigating and dilating lacrimal duct after surgery （the third postoperative day , then a week after the operation, followed by once a week till a month）. The curative effect in each group was compared. Results Within 1 month after surgery the curative rate between both group A, B and group C, D, showed no statistical difference （P〉0.05 ）, while 3 month after surgery the curative rate of group A, B were 57.9%, 83.3%, respectively , and that of group C, D were 47.4%, 72.5% respectively, the difference between both group A, B and group C, D were statistically significant （P〈0.05 ）. Conclusion Proper irrigating and dilating lacrimal duct may reduced the curative rate after endolacrtmal recanalisation surgery by KTP laser, while frequently irrigating and dilating lacrimal duct may make the curative rate decrease due to the scar of lacrimal duct membrane.
