摘 要:目的:评估B型钠尿肽(BNP)对心力衰竭(HF)诊断及预后的临床价值。方法:对196例HF患者和28例健康对照者进行BNP测定,并与临床诊断以及患者30天内是否发生HF终点事件相比较,分析BNP与HF分级及预后的关系。结果:BNP浓度与HF程度相关,BNP≥100pg/ml诊断HF的灵敏度为94.0%,特异性为72.2%,阴性预测值为88.5%,阳性预测值为79.5%;接收者运行特征(ROC)曲线下面积为0.921;并能预示HF患者的预后。结论:BNP测定有助于临床医师客观判断病情及预后,是很好的心脏功能监测指标。Objective: To assess the importance of B-type natriuretic peptide(BNP) on diagnosis and prognosis in heart failure(HF).Method: Subjects consisted of 196 HF patients and 28 normal controls.BNP levels were analyzed.The results were compared with the diagnosis and the frequency of clinical end point in 30 days to explore the relationship between BNP level and HF degree and prognosis.Results: BNP level correlated to the degree of HF,BNP ≥100pg ml had 94.0 sensitivity and 72.2 specificity for HF with 79.5 positive predictive value and 88.5 negative predictive value.The area under the receiver operating characteristic curve is 0.921.BNP could predict the prognosis in HF.Conclusion: BNP concentration is a good monitor index for heart function,and is useful in the diagnosis and prognosis in HF.
